Support Whole-Person Care as a Nurse Care Manager in Our Behavioral Health Healthcare Home. We’re seeking a Nurse Care Manager to join our integrated care team and make a meaningful impact on the health of individuals with behavioral health needs. In this role, you’ll coordinate physical health support for clients in our Healthcare Home program—bridging the gap between behavioral and medical care to help clients live healthier, fuller lives. Monday-Friday Schedule. Requirements

  • High School/GED required
  • Practical Nursing or Associate of Nursing degree required
  • Licensed Practical Nurse (LPN) required
  • A valid driver's license and agency established minimum automobile coverage required

Nice To Haves

  • Bachelor’s degree in nursing preferred
  • Registered Nurse (RN) preferred
  • Proficiency in Microsoft Office Suite and Electronic Medical Records preferred

Responsibilities

  • Coordinate care for up to 200 clients, completing health screenings and identifying physical health risks
  • Support wellness planning by setting physical health goals and promoting healthy lifestyle changes
  • Educate clients on chronic condition management, appropriate healthcare usage, and health prevention
  • Complete screenings (e.g., metabolic syndrome), medication reconciliations, and health consultations
  • Collaborate with PCPs, specialists, and community partners to ensure holistic, person-centered care
  • Participate in interdisciplinary team meetings and provide nursing support across clinic locations as needed
