About The Position

If you are looking to step away from traditional hospital shifts and transition into a consultative, office-based setting, this role offers a highly stable daytime schedule without nights, weekends, or on-call demands. In this role, you will partake in our team-based approach for clients managing severe mental illness alongside chronic, complex physical health conditions. You will collaborate daily with a multidisciplinary team consisting of a Clinical Lead, Nurse Care Manager, Team Leads, Case Managers, a Peer Support Specialist, and two Psychiatric Consultants. This position is focused on medical consultation, interdisciplinary collaboration, health education, and client advocacy.

  • Current CPR/Basic Life Support (BLS) certification.
  • Valid driver's license, a good driving record, and reliable transportation (essential for providing client advocacy at local doctor's appointments).
  • Comfort navigating Electronic Health Records (EHR) and standard office software for data-driven care coordination.

  • Conduct health education sessions for clients and offer vital health-literacy guidance to our non-medical staff.
  • Utilize our Electronic Health Record (EHR) to track client progress, analyze health outcomes, and translate medical data into actionable interdisciplinary care plans.
  • Advocate for client access to crucial external resources and coordinate seamlessly with external community health providers.
