About The Position

The Nurse Care Coordinator – Ambulatory performs duties consistent with the mission, vision and values of Indiana Lakes Accountable Care Organization (ILACO) and Goshen Health. This position exists under the supervision of the Ambulatory Care Coordination Manager. The incumbent works in collaboration with the primary care physician, staff and the patient to ensure care coordination occurs across the continuum of care. Focuses on high risk patient care coordination and care transitions (hospital stays/30 day readmissions/emergency department (ED) visits) communicating the patient plan of care with all members of the team. Responsible for coordinating a wide range of patient services, education and information and partners with the patient/caregiver in the plan of care encouraging self-management.

Requirements

  • Active Indiana license as a Registered Nurse.
  • Three to five years of clinical experience.
  • Clinical skill knowledge.
  • Strong analytical and critical thinking skills.
  • Understanding of population health principles.
  • Demonstrate understanding/application of research methodology, statistical analysis.
  • Understanding of medical terminology.
  • Ability to take initiative and meet objectives.
  • Knowledge of computer use and various data base software programs.
  • Ability to adapt and be flexible with frequent change.
  • Strong organizational and interpersonal skills.
