Requirements

  • High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
  • Current State of Hawaii Certified Nurse Aide certificate.
  • Current First Aid and CPR certificate.
  • Proficiency in using a computer, which may include using such software as Vision or Microsoft Office Suite (Excel, Outlook, PowerPoint, Word) and other applicable software applications.

Responsibilities

  • Answers call lights to determine residents’ needs.
  • Assists residents with activities of daily living (ADLs) such as: Bathing; Eating; which includes set-up, serving and collecting food plates and utensils, Feeding residents who require assistance; Dressing; Oral hygiene; and Grooming as needed.
  • Provides assistance with toileting and provides appropriate peri-care for incontinent residents.
  • Assists residents to turn, reposition, and transfer, which may be performed by the NA without assistance, with a two-person assistance, or by using a lift machine in accordance with the service plan.
  • Transports residents with ambulation to and from their room to appropriate areas for activities and meals.
  • Takes and records ADLs and chargeable time/units, Vitals (temperature, blood pressure, pulse, weight, and respiration rates), Intake and output of food and fluid, Transfers (one or two person) and ambulation status, Mood/behaviors when resistant to care, Pain level assessment (if applicable)
  • Provides companion services according to service plan.
  • Observes and reports to Licensed Staff all changes in condition (including significant changes), behavior or whereabouts that may require intervention.
  • Monitors and supervises residents as assigned.
  • Adheres to assigned work schedules and accurately uses the time and attendance system.
  • Participates in Continuous Quality Improvement and/or Quality Assurance Programs.
  • Interacts with residents in accordance with AFCs’ mission, core values, mindsets and guideposts.
  • Exercises caution and good safety judgment in uncertain conditions or situations, and report and/or correct hazards, potential or actual.
  • Ensures that all residents’ rights and privacy are protected and enforced, and establishes and maintains a close liaison with families.
  • Attends mandatory in-service programs.
  • Complies with annual employee health screening in accordance with policies and procedures.
  • Participates and assists during emergency drills and situations such as fire, fire alarms, hurricanes, and other possible emergencies as outlined in the Disaster Response Playbook and/or the disaster policy.
