About The Position

The NPST Nurse Practitioner is responsible for practicing the nursing profession ethically and according to the Scope and Standards of Practice of the Texas Board of Nursing. This role involves Provider-level care coordination for all ISNP Members, as outlined in the Texas Independence Health Plan (ISNP) Model of Care. The NP is expected to be present in their assigned facilities during customary working hours (approximately 8:00 am – 5:00 pm) to ensure adequate Provider-Level coverage and to allow time for documentation and rotational on-call responsibilities. The position requires maintaining a high level of professionalism, attending daily morning meetings, and performing duties within the scope of practice allowed by prescriptive authority agreements. The NP will report to the Clinical Operations Manager (COM) and collaborate with facility staff regarding patient care. This role provides proactive, preventative, and maintenance care for the medical, emotional, and behavioral conditions of Plan Members, including clinical care, education to members, families, and facility staff, and adherence to ISNP Model of Care Provider metrics. Participation in an on-call rotation is required for after-hours continuity of care. Efficient navigation and data entry into Electronic Medical Record Systems are essential, as is participation in ongoing staff education and Quality Improvement initiatives. Professional communication with families and staff, review of medications and diagnostic tests, and evaluation/treatment of chronic illnesses and acute exacerbations are key responsibilities. The NP will also provide education on Advance Care Planning and review consultant recommendations. All duties must be congruent with the ISNP Plan, and the NP must maintain adequate Medical Malpractice Insurance.

Requirements

BSN, MSN
Current Board Certification
Licensed as both a Registered Nurse and an Advanced Practice Registered Nurse in the state of Texas.
Maintains an up-to-date Board Certification in his/her specialty.
Responsible for maintaining a Texas Statutory Prescriptive Authority Agreement with attending Physicians and/or Medical Directors.
Responsible for maintaining Texas Board of Nursing ongoing educational requirements for licensure and re-certification.
Will comply with all State and Federal guidelines required to practice as a licensed Advanced Practice Registered Nurse, including maintaining an active RN and APRN license.
Will maintain Professional Medical Malpractice Liability Insurance.

Responsibilities

Practicing the nursing profession ethically, dutifully, and according to and within the Scope and Standards of Practice of the Texas Board of Nursing.
Provider-level care coordination, as outlined in the Texas Independence Health Plan (ISNP) Model of Care, for all ISNP Members.
Maintaining a high measure of professionalism in dress and actions that appropriately reflect the mission and policies of the organization and facilities.
Attending and participating in daily morning meetings when in facilities.
Accountable for own clinical and professional actions and performing duties within the appropriate scope of practice allowed by respective prescriptive authority agreements with attending physicians.
Responding timely to all Compliance requests.
Reporting to the Clinical Operations Manager (COM), maintaining communication with COM and facility staff regarding Plan specific to facility and/or patient/family concerns.
Provider-Level health care delivery and preventative care management for specific Texas Independence Health Plan ISNP Plan Members (Plan Members) who are assigned to collaborating physicians in assigned practice facilities.
Providing proactive, preventative/maintenance care for the Plan Members’ medical, emotional, and behavioral conditions.
Providing clinical care and education to Plan Members, as well as education to the family and Nursing Facility staff.
Adhering to and being accountable to all ISNP Model of Care Provider metrics and goals.
Participating in an on-call rotation to ensure continuity of care for Plan Members after hours, weekends and holidays.
Maintaining reasonable and appropriate communication with the facility staff for questions, notifications, and family concerns to assist in providing optimal clinical care.
Efficiently navigating and operating a laptop computer and entering data and documentation daily into designated Electronic Medical Record Systems in a timely manner.
Participating in ongoing staff education and reasonably assisting, as requested, with Quality Improvement at the facilities/communities.
Communicating professionally in face to face conversations with families and staff.
Reviewing medications, orders, and diagnostic tests, and responding to abnormalities appropriately, within BON approved scope of practice.
Providing ongoing evaluation and treatment options of chronic illnesses, and care for acute exacerbations of chronic illnesses as they occur.
Providing ongoing evaluation and education to patients and families on Advance Care Planning (Goals of Care and Anticipatory planning).
Reviewing and responding to consultant recommendations such as psychiatric, dietary, rehabilitative therapies, and pharmacy.
Performing all other duties congruent with the ISNP Plan as assigned.
Maintaining adequate Medical Malpractice Insurance (generally expected to be $1m/$3m).
