About The Position

Coordinates health care for select patient populations or designated services across the continuum of care. The Nurse Coordinator coordinates patient contact and education and prepares patients and families for procedures or tests in collaboration with the healthcare team. This is a full-time outpatient opportunity in Sunset Hills, working five 8-hour shifts Monday through Friday from 8:00 a.m. to 4:30 p.m. There are no weekends, holidays, or on-call duties.

Requirements

• Nursing diploma or associate degree in nursing.
• Two to five years of experience.
• Registered Nurse (RN) license.
• EPIC experience is required.

Nice To Haves

• Non-oncology infusion experience.
• Bachelor’s degree in nursing.

Responsibilities

• Assess and evaluate patients’ physical, psychosocial, and emotional needs to facilitate care.
• Coordinate the delivery of safe and appropriate patient care in accordance with evidence-based clinical practice.
• Monitor care and data to optimize patient outcomes.
• Facilitate education for patients and families.
• Act as a resource to the healthcare team.
• Ensure actions support the overall goals of the business.
