About The Position

AIDS Healthcare Foundation (AHF) is the largest specialized provider of HIV/AIDS medical care in the nation, founded in 1987. A Non-Medical Care Manager contributes to AHF by utilizing strong experience in social work, understanding the social deterrents of this population, and a thorough understanding of the Ryan White Care Act Program within their specific area. This role involves working with providers and the healthcare center team to ensure Ryan White patients receive necessary support and access to resources, including education and assistance regarding program eligibility. Working directly within the Healthcare Center, the manager will complete Health Risk/Needs Assessments and develop Service/Action/Care Plans related to psychosocial needs and goals. They will understand mental health needs and basic needs like housing, food, and clothing, develop relationships with relevant programs and agencies, and facilitate referrals and support for patients lacking these services. This supports patients in understanding their health status to achieve healthcare goals. The Non-Medical Care Manager enhances AHF's reputation by using case management experience to meet members where they are, identify strengths and growth opportunities, and assist with realistic goal setting. Through collaboration and engagement, they bring the essence of the Ryan White Care Act – the safety net for uninsured HIV/AIDS patients – to a vulnerable population. Their work directly contributes to AHF’s success in meeting Ryan White contract deliverables and measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services. The role involves being a passionate advocate for top initiatives and ensuring AHF core values and mission are upheld. The Non-Medical Care Manager works as part of a close-knit team, collaborating with providers and patients to facilitate the implementation of optimum care plans for non-medical needs, providing brief interventions focused on long-term achievable goals towards viral suppression, better health outcomes, and quality of life.

Requirements

  • Bachelor degree in health or human services-related field

Nice To Haves

  • Minimum of 1 year Case Management experience
  • Knowledge of Ryan White Services

Responsibilities

  • Utilize strong experience in social work, understanding the social deterrents of this population, and a thorough understanding of the Ryan White Care Act Program within their specific area.
  • Work with providers and healthcare center team so that every Ryan White patient receives the support they require and access to available resources.
  • Provide education and assistance so that eligibility for the Ryan White program is understood.
  • Complete Health Risk/Needs Assessments.
  • Develop Service/Action/Care Plans related to psychosocial needs and goals.
  • Draw on experience and skills in understanding mental health needs and basic needs such as housing, food, and clothing.
  • Develop relationships with programs and agencies serving those needs.
  • Facilitate referrals, support, and assistance for patients who lack those services.
  • Support the patient’s ability to better understand their health status so that they can work to achieve their healthcare goals.
  • Meet members where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting.
  • Bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population.
  • Directly contribute to AHF’s success in meeting the deliverables of our Ryan White contract.
  • Contribute to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

  • Collaborate with the provider and patient and facilitate the shift from leaving the exam room to implementing the optimum plan of care for these non-medical needs and putting it into action.
