About The Position

NYU Langone Hospital—Brooklyn is seeking a Night Shift Pharmacist to join their team. This role is responsible for the safe preparation, compounding, and dispensing of medications, optimizing drug therapy, and working as a member of the inpatient care team. The pharmacist will also be involved in covering satellite pharmacy operations, preparing IV products, distributing controlled substances, providing drug information, and ensuring the safe management of hazardous medications.

Requirements

• Bachelor of Science degree in Pharmacy or Doctorate of Pharmacy from an accredited college of pharmacy or equivalence.
• Licensed to practice in NY and license in good standing.
• Active and current NY pharmacy license.
• Qualified candidates must be able to effectively communicate with all levels of the organization.

• Competency assessments in areas of population specific pharmacotherapy, aseptic technique, anti-infective, anti-coagulation therapy.

Responsibilities

• Safely preparing, compounding, and dispensing medications to patients in accordance with laws, regulations, and hospital policy.
• Optimizing drug therapy by identifying, resolving, and preventing drug-related problems and improving therapeutic outcomes.
• Working independently within established pharmacy policies and procedures as a member of the inpatient care team.
• Directing and coordinating the activities of a pharmacy service area.
• Covering satellite pharmacy operations, including preparing IV products, distributing controlled substances, and providing drug information.
• Ensuring knowledge of safe management and handling of hazardous medications.
• Ensuring medication therapies are cost-effective and safe.
• Distributing and maintaining controlled substances to Automated Dispensing Cabinets (ADCs), including restocking, destocking expired/recalled substances, assigning/unassigning/removing/returning items, and receiving substances into the cage.
• Maintaining inventory control, safety, and security of controlled substances.
• Ensuring proper record keeping and maintenance of records for all narcotic transactions.
• Informing the pharmacy director of controlled substances to be ordered.
• Dispensing and verifying compounded narcotic IV products and maintaining inventory control.
• Reporting discrepancies to the pharmacy manager immediately.
• Ensuring overall security and control of the narcotic cage.
• Conducting narcotic inventory as required and reporting discrepancies.
• Assigning patient's own controlled substance(s) bins in the ADC.
• Collecting and managing narcotic returns via External Return Bins (ERBs).
• Checking and removing expired/near expired narcotics from all ADCs.
• Preparing ambulance boxes.
• Completing assignments under the direction of the clinical pharmacist or clinical staff.
• Attending and participating in daily medical and multi-disciplinary rounds.
• Attending and participating in daily patient discharge counseling and patient education.
• Participating in the training of medical, nursing, and allied health professionals.
• Covering in the main pharmacy or satellite pharmacy for routine staffing, rotation, or break coverage.
• Verifying Adult and Pediatric/Neonatal Total Parenteral Nutrition (TPN) orders.
• Checking emergency trays, emergency carts, ambulance boxes, etc.
• Working in the sterile products area, ensuring accuracy and sterility of compounded products based on USP 797 regulations.
• Preparing and verifying Adult IV orders.
• Preparing and verifying Neonatal and Pediatric IV orders.
• Ensuring the safest environment possible and compliance with USP 797 regulations.
• Supervising and monitoring IV technicians.
• Supervising and informing the pharmacy manager of sanitation and maintenance issues of the IV clean room area.
• Double checking all IV and chemo orders.
• Demonstrating knowledge and skills necessary to provide care based on physical, psychosocial, education, safety, and age-related criteria.
• Interpreting and implementing departmental policies and procedures on the assigned shift.
• Accurately interpreting physician orders for completeness and appropriateness.
• Filling medication cassettes.
• Monitoring patient drug therapy, including interactions.
• Documenting and reporting all clinical interventions and adverse drug events.
• Ensuring step-down medication therapies have been appropriately assessed for conversion.
• Communicating therapeutic or toxicity concerns to the prescriber immediately.
• Utilizing all available resources to reach clinically sound decisions.
• Initiating action to solve problems and assuming responsibility.
• Verifying the accuracy of all work completed by supportive personnel.
• Submitting assigned monthly inspection reports and equipment maintenance logs.
• Formulating and compounding special dosage forms not commercially available.
• Attending, participating in, and conducting continuing education programs and conferences.
• Maintaining skill and competency level to comply with departmental and hospital Quality Assurance standards.
• Participating in department quality improvement efforts.
• Assisting with the training of clinical pharmacy externship/internship students.
• Providing staff coverage in other areas of the department when appropriately trained.
• Demonstrating understanding of cultural competence.
