With the focus on the patient as the primary beneficiary, provide leadership, clinical, educational, informational, and distribution functions on the night shift. Prioritize the workload and organize the workflow of pharmacy technical personnel on the night shift, taking into account available resources. Follow departmental procedures for checking the accuracy of the pharmacy technical personnel in assigned areas. May conduct in-service programs for medical, pharmacy and nursing staff. May act as the liaison between pharmacy services and other departments and/or participate on committee meetings as a department representative. Identify performance improvement opportunities and make recommendations of ways to improve clinical and distributive services. Proactively work to improve the medication process. Where applicable, use effective patient education techniques to provide counseling to patients and caregivers including information on drug therapy, adverse effects, compliance, appropriate use, handling and drug administration. May respond to questions from patients regarding their medications. Identify the requester's actual drug information need. Employ a systematic, efficient, and thorough procedure of retrieving drug information. Effectively manages pharmacy operations on the night shift in the absence of an on-site supervisor; seeks assistance from management as appropriate. Actively participates in employee performance and competency assessment. Coaches and counsels staff and documents as required. Assure the appropriateness of a medication order including age-specific and organ-specific factors, allergy checking, duplicate checking and drug interaction checking before preparing or permitting the distribution of the first dose. Maintain the accuracy of patient profiles according to Emory Healthcare's Policies and Procedures. Utilize EMAR Summary to help with scheduling doses, avoiding duplicated doses and correctly initiating change in doses. Prepare drug products using appropriate techniques and following the Emory Healthcare's Policies and Procedures. Dispense drug products in accordance with established policies and procedures. Exercise skill as a team member in the management of medical emergencies by attending codes as assigned. Ensure that policies and procedures related to drug distribution are followed. Recommend appropriate alternatives related to non-formulary requests and shortages/recalls. Ensure the accuracy of records pertaining to controlled substances, medication compounding, prepackaging, and sterile product preparation. Collect and organize all patient-specific information (medication work-up) needed to assure rational, safe therapy for patients on assigned areas. This shall include prevention, detection and resolution of medication-related problems including: Kinetics Renal dosing Anticoagulation Protocols and guidelines Medication reconciliation Core measures Refer to the Identification, Prevention or Resolution of Drug Related Problems policy Identify the presence of medication therapy problems in patients current medication therapy; develop a plan to prevent or resolve the problem in collaboration with members of the healthcare team. Utilize nomograms and other departmental guidelines when providing medication-related recommendations. Document those pharmaceutical care activities which require documentation including adverse drug reactions, adverse drug events and interventions. Write effective notes when documenting patient care in EeMR. Works with manager to formulate plan for professional development. Attends educational in-services as appropriate. Participates in professional activities and organizations to maintain knowledge of current trends, practices, and developments.
