About The Position

NYU Langone Hospital—Brooklyn is seeking a Night Pharmacist to join their team. This role is responsible for the safe preparation, compounding, and dispensing of medications, optimizing drug therapy, and working as a member of the inpatient care team. The position involves covering satellite pharmacies, preparing IV products, distributing controlled substances, providing drug information, and ensuring the safe management of hazardous medications. The pharmacist will also be responsible for pharmacoeconomic considerations, narcotic management, clinical support, order checking and verification, and ensuring compliance with USP 797 regulations. This role requires a Bachelor of Science degree in Pharmacy or Doctorate of Pharmacy, an active and current NY pharmacy license, and the ability to effectively communicate with all levels of the organization. Board Certification in a relevant area is highly desirable. The position involves night shifts and requires completion of various safety and competency training upon orientation and annually thereafter.

Requirements

  • Must have a Bachelor of Science degree in Pharmacy or Doctorate of Pharmacy from an accredited college of pharmacy or equivalence.
  • Must be licensed to practice in NY and license in good standing.
  • Active and current NY pharmacy license.

Responsibilities

  • Safely preparing, compounding, and dispensing medications to patients in accordance with laws and regulations.
  • Optimizing drug therapy by identifying, resolving, and preventing drug-related problems and improving therapeutic outcomes.
  • Working independently within established pharmacy policies and procedures as a member of the inpatient care team.
  • Directing and coordinating the activities of a pharmacy service area.
  • Covering satellite pharmacy, preparing IV products, distributing controlled substances, and providing drug information.
  • Ensuring safe management and handling of hazardous medications.
  • Ensuring that medication therapies are cost-effective and safe.
  • Distributing and maintaining controlled substances to all Automated Dispensing Cabinets (ADCs).
  • Restocking of controlled substances.
  • Destocking of expired/recall controlled substances.
  • Assigning/Unassigning/Removing/Returning controlled items in the ADC.
  • Receiving controlled substances into the cage.
  • Maintaining inventory control, safety, and security of controlled substances.
  • Ensuring proper record keeping and maintenance of narcotic transaction records.
  • Ensuring proper narcotic inventory control and informing the pharmacy director of controlled substances to be ordered.
  • Dispensing and verifying compounded narcotic IV products and maintaining inventory control.
  • Reporting discrepancies discovered to the pharmacy manager immediately.
  • Ensuring overall security and control of the narcotic cage.
  • Performing ad hoc restock and supply of controlled substances.
  • Conducting narcotic inventory as required and reporting discrepancies.
  • Assigning patient's own controlled substance(s) bins in the Automated Dispensing Cabinet.
  • Collecting and managing narcotic returns via External Return Bins (ERBs).
  • Checking and removing expired/near expired narcotics from all ADCs.
  • Preparing ambulance boxes.
  • Completing assignments under the direction of the clinical pharmacist or clinical staff.
  • Attending and participating in daily medical and multi-disciplinary rounds.
  • Attending and participating in daily patient discharge counseling and patient education.
  • Participating in the training of medical, nursing, and allied health professionals.
  • Covering in the main pharmacy or satellite pharmacy for routine staffing, rotation, or break coverage.
  • Verification of Adult and Pediatric/Neonatal Total Parenteral Nutrition (TPN) orders.
  • Checking emergency trays, emergency carts, ambulance boxes, etc.
  • Working in the sterile products area, ensuring accuracy and sterility of all compounded products based on USP 797 regulations.
  • Preparation and verification of Adult IV orders.
  • Preparation and verification of all Neonatal and Pediatric IV orders.
  • Ensuring the safest environment possible.
  • Ensuring compliance with USP 797 regulations (temperature, humidity, room pressure differential, aseptic techniques, etc.).
  • Supervising and monitoring IV technicians.
  • Supervising and informing the pharmacy manager of sanitation and maintenance issues of the IV clean room area.
  • Double checking all IV and chemo orders.
  • Demonstrating knowledge and skills necessary to provide care based on physical, psychosocial, education, safety, and age-related criteria.
  • Interpreting and implementing all departmental policies and procedures on the assigned shift.
  • Completely and accurately interpreting physician orders for completeness and appropriateness.
  • Filling medication cassettes.
  • Monitoring patient drug therapy including drug-drug, drug-food, drug-herbal, and drug-pregnancy/lactation interactions.
  • Documenting and reporting all clinical interventions and adverse drug events.
  • Ensuring that all step-down medication therapies have been appropriately assessed for conversion.
  • Communicating any therapeutic or toxicity concerns to the prescriber immediately.
  • Utilizing all available resources to reach clinically sound decisions.
  • Initiating action to solve problems and assuming responsibility.
  • Verifying the accuracy of all work completed by supportive personnel.
  • Submitting assigned monthly inspection reports and equipment maintenance logs.
  • Formulating and compounding special dosage forms not commercially available.
  • Attending, participating in, and conducting continuing education programs and conferences.
  • Maintaining skill and competency level to comply with departmental and hospital Quality Assurance standards.
  • Participating in department quality improvement efforts.
  • Assisting with the training of clinical pharmacy externship/internship students.
  • Providing staff coverage, when needed, in areas of the department that the individual has been appropriately trained in.
  • Performing all other responsibilities as assigned.

  • Providing drug information services as required.

  • Monitoring workflow processes and initiating change when necessary.
  • Ensuring filling of medication orders so that scheduled medications are administered to patients in a timely manner.
  • Ensuring overall aspects of department security and control.
  • Ensuring short supply medications are communicated to management.
  • Ensuring compliance with all regulatory standards of practice.
