About The Position

Yale New Haven Health System is seeking a full-time Advanced Practice Provider (APP) to join the Hospitalist team at Gaylord Hospital in Wallingford, CT. In this role, you will be a key member of the Hospitalist team, providing high-quality, timely care to patients across the hospital. It is a full-time position, rotating 12-hour shifts, on a 3, 3, 4-night schedule.

Requirements

  • Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
  • Active PA or NP Certification by the National Commission of Certification of Physician Assistants (NCCPA), or by the American Nurses Credentialing Center (ANCC), or the American Academy of Nurse Practitioners Certification Board (AANPCB).
  • Connecticut State Physician Assistant or Nurse Practitioner License, ACLS/BLS Certified, when indicated for position profile DEA License.
  • APRNs and Pas must be appointed to the Affiliated Medical Staff of a YNHHS hospital and go through the Medical Staff credentialing process. To be eligible, evidence of current competence to practice as an APRN or PA must be provided. This includes obtaining appropriate reference letters from physicians and other practitioners during the Medical Staff appointment process.
  • Additional licensure, certifications and/or continuing education credits may be required during employment based on position and area of specialty.

Nice To Haves

  • Clinical working experience as an NP/PA in the specialty/service line is preferred but not required.

Responsibilities

  • Provide comprehensive inpatient medical care, including obtaining medical histories, performing physical examinations, and delivering treatment within the APP scope of practice.
  • Develop and manage diagnostic and treatment plans, such as ordering labs, imaging, consults, referrals, and prescribing medications in accordance with state and federal regulations.
  • Monitor patient progress and adjust treatment plans, evaluating physiological and behavioral responses and collaborating closely with attending physicians and clinical staff.
  • Respond to urgent clinical issues and perform approved procedures aligned with demonstrated competencies and hospital‑approved privileges.
  • Participate in interdisciplinary team activities, including unit meetings, family conferences, quality improvement initiatives, and administrative committees as needed.
