About The Position

Oak Street Health's NP Fellowship in Adult Primary Care, in collaboration with the University of Michigan School of Nursing, is a 12-month paid program designed to equip new Nurse Practitioners with the skills and support needed to become confident and competent clinicians for older adults within a value-based care model. Fellows will build their own patient panels, learn from various preceptors, and engage in didactic content that complements their graduate training. The program aims to prepare fellows for a year of employment at Oak Street Health post-fellowship, with the hope of long-term careers improving community health. Two cohorts begin annually, in September and March.

Requirements

  • Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their programs between December 2024 and May 2026.
  • Must currently reside in, or be willing to relocate to, the Fellowship program location for at least the next two years.
  • Have not yet worked as an NP.
  • Minimum of one year experience, within the last five years, working as an adult RN.
  • Minimum GPA of 3.0 in NP program.
  • Have passed, or in the process of, passing NP board examinations by 6/6/2026.
  • Currently holds or will hold by 8/3/2026 an active, non-probationary state NP license.
  • A genuine passion for adult primary care.
  • A supportive attitude toward our patient population of older adults.
  • Embrace teamwork and the opportunity to collaborate with brilliant colleagues.
  • Want to be a part of an innovative model focused on empirically guided population health.
  • One (1) reference from a current or recent supervisor.
  • Two (2) references from a Preceptor, Program Director, or Faculty member.
