About The Position

Oak Street Health is seeking Nurse Practitioners (NPs) for its New Graduate Nurse Practitioner Fellowship in Adult Primary Care. This 12-month paid fellowship, in collaboration with the University of Michigan School of Nursing, is designed to equip new NPs with the skills and support needed to become confident and competent clinicians for older adults within a value-based care model. Fellows will build their own patient panels, learn from various preceptors, and engage in advanced didactic content. The program aims to foster long-term careers at Oak Street Health, contributing to the improvement of communities nationwide. Cohorts begin in September and March annually.

Requirements

• Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their programs between December 2024 and May 2026.
• Must currently reside in, or be willing to relocate to, the Fellowship program location (Soundview - 1555 Westchester Ave, Bronx, NY 10472) for at least the next two years.
• Have not yet worked as an NP.
• Minimum of one year experience, within the last five years, working as an adult RN.
• Minimum GPA of 3.0 in NP program.
• Have passed, or in the process of, passing NP board examinations by 6/6/2026.
• Currently holds or will hold by 8/3/2026 an active, non-probationary state NP license.
