About The Position

This position is responsible for planning, directing, coordinating, and providing high-quality, individualized patient care using the components of the nursing process. The role involves performing assessments, developing and implementing care plans, evaluating patient outcomes, and collaborating with healthcare teams. Key responsibilities include ensuring patient safety, providing patient and family education, managing emergency situations, and maintaining patient rights and dignity. The nurse will care for patients across the healthcare continuum, including end-of-life care, and must demonstrate knowledge of age-specific care for adolescent through geriatric populations. The position also requires effective communication, adherence to organizational policies, and the ability to function as a Team Leader/Charge Nurse.

Requirements

  • Graduate of a RN Training Program
  • Minimum one-year recent experience in an acute care setting preferred
  • Ability to communicate effectively verbally and in writing.
  • Must be able to work in a union environment.
  • Current California RN license
  • AHA BLS card
  • AHA ACLS card
  • EKG certification
  • NIHSS Certification
  • Current Los Angeles County Fire Card required (within 30 days of hire).
  • Assault Response Competency (ARC) (within 30 days of hire).

Responsibilities

  • Planning, directing, coordinating, and providing high-quality, individualized patient care using the components of the nursing process.
  • Performing initial assessments and ongoing reassessments in a timely manner, incorporating cultural and ethnic factors, and differentiating between normal and abnormal findings.
  • Incorporating the functional/developmental age into assessment/reassessment for the adolescent through geriatric patient population.
  • Developing, implementing, and evaluating the nursing aspects of the trans-disciplinary care plan using hospital and regulatory standards of care.
  • Developing an individualized patient care plan in conjunction with the patient/family and determining interventions based on assessment/reassessment findings.
  • Evaluating patient outcomes on an ongoing basis and establishing priorities for patient care services.
  • Accurately documenting patient care activities in the medical record.
  • Planning, guiding, directing, and coordinating patient care across the continuum using a case management approach.
  • Effectively communicating with physicians, staff, and other disciplines.
  • Assessing and responding appropriately to ensure patient safety based on the physical environment, equipment, and patient behavior.
  • Participating in patient/family teaching by reviewing educational materials and coaching nurses and hospital staff on education interventions.
  • Providing care in emergency situations and following established emergency procedures and practices.
  • Communicating and following the organizational chain of command for notification of patient care/service issues.
  • Maintaining knowledge of therapeutic patient care issues/problems and determining measures and interventions to promote, maintain, and/or restore health for the adolescent through geriatric patient population.
  • Providing appropriate care through the healthcare continuum, including end-of-life care.
  • Assuring that the rights of all patients are respected and maintained by allowing for privacy and dignity.
  • Determining the presence and type of Advance Directive, if applicable.
  • Ensuring patient and/or family involvement in the decision-making process related to patient care, end of life, and ethical issues.
  • Reporting suspected cases of abuse or neglect relative to the adolescent through geriatric patient population.
  • Demonstrating knowledge of age-specific care for adult and geriatric populations, including safety measures, physiological values, skin integrity, behavior, motor skills, and risk factors.
  • Communicating effectively in a clear, concise, and understandable manner.
  • Identifying patient/family educational needs and initiating and providing individualized teaching.
  • Evaluating and documenting the effectiveness of patient/family teaching.
  • Demonstrating effective Team Leader/Charge Nurse skills.
  • Practicing Universal Precautions.
  • Demonstrating flexibility in the work setting with changing patient assignments, varying staffing levels, and patient care environments.
  • Completing work on time.
