About The Position

Corewell Health operates over 300 outpatient clinics across Michigan, offering a wide array of care options to the communities they serve. These clinics encompass over 40 different types of primary care and specialty practices, providing opportunities for staff to explore various interests or specialize in a particular area. The Neurosurgery Nurse Navigator is responsible for delivering quality education, follow-up, and emotional support to patients and their families. This role involves case managing the patient population across both inpatient and outpatient settings, with a focus on measuring outcomes to ensure maximum continuity of care between these environments. Additionally, the Nurse Navigator will manage projects and programs related to clinical management and perform quality improvement activities to assess program processes and outcomes.

Requirements

• Bachelor's Degree Nursing
• 5 years of previous nursing or CDE clinical experience
• Prior teaching with patients of all ages
• LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
• CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required or CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required
• CRT-Oncology Certified Nurse - ONCC Oncology Nursing Certification Corporation required within 1 year
• CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 120 Days required or CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 120 Days required
