The Concussion Nurse Navigator works as a key member of the Concussion Program Team serving as the point person and liaison for the community. This position serves as a consistent contact for and coordinator of multidisciplinary clinical care delivery to achieve exceptional patient/family-centered care and clinical outcomes for an identified population of concussion patients. The Nurse Navigator collaborates with other members of the team to identify the caseload of patients, who can best benefit from Navigation services, and to build and execute a care pathway for each population of potential patients. The Concussion Nurse Navigator is responsible for supporting the liaison work, education, and performance feedback to schools and universities in the community. The focus of this role is on relationship building and collaboration between these external networks and the Concussion Program at Yale New Haven Hospital. The Nurse Navigator ensures patient appropriateness and preparedness for medical and/or surgical treatment through education and psychosocial support, while building communication pathways for referral to the program. The Nurse Navigator focuses on: (1) navigating patients/families across care settings according to the established clinical pathways, (2) ensuring that the patient/family have appropriate information, understand and have the capacity and tools to adhere to the therapeutic plan, and (3) bridging among and between various disciplines, services and points of care encountered by the patient /family. The Nurse Navigator assists the clinical and leadership team with program development, expansion and successful achievement of programmatic goals.

Requirements

  • Bachelors Degree in Nursing required
  • Minimum of 3 years of experience in nursing; neurosciences preferred.
  • Experience in a combination of the following will be considered: outpatient clinical care, community nursing, population health, clinical process improvement, outcomes management, case management, and physician relations.
  • Must possess understanding of clinical processes, organizational dynamics and process improvement.
  • Must have excellent assessment and communication skills.
  • Effective problem solving and capacity to participate in both verbal and written communication.
  • Ability to negotiate and flex to the requests and needs of various community programs.

Nice To Haves

  • Masters Degree in Nursing or Healthcare preferred

Responsibilities

  • Collects data for new patients using established community assessment guidelines prior to initial visit to assess patient/family current and projected future needs
  • Develops community education plans based on needs assessment findings relevant to teaching learning topics, styles and the identification of special needs and preferences
  • Uses findings of assessment data, team-based interactions and established triggers to initiate referrals for supportive care (social work, nutrition, rehabilitation, psychosocial services) and community agency assistance (home care, support groups); monitors and reports response to consultative interventions
  • Oversees Concussion Program Team performance by analyzing accuracy and completeness of data compiled on new patients and observing patient/family interactions; provides feedback to Manager
  • Supports the Neurosciences leadership team in ensuring that Patient Experience standards reflect the YNHH Service Excellence Pledge and policy
  • Advocates for the professional nurse as a key member of the inter-professional team
  • As requested, assists with data abstraction and statistics
