About The Position

This role exists to change the trajectory of chronic kidney disease, dialysis, and transplant care—not just to fill a clinic schedule. You will be the physician patients trust when the plan has to work in real life, and the colleague the care team relies on when decisions cannot wait.

Requirements

  • M.D. or D.O. with board certification or active eligibility in Nephrology, and an unrestricted license to practice.
  • Demonstrated ability to manage CKD through dialysis and transplant evaluation, including acute inpatient consults.
  • Comfort leading multidisciplinary rounds and giving referring clinicians concise, actionable recommendations.
  • Habit of documenting plans that other clinicians can execute without a follow-up call.
  • Commitment to equitable access for patients who face transportation, language, or coverage barriers.

Responsibilities

  • Build and sustain a panel of patients with CKD, ESRD, hypertension, and electrolyte disorders, with care plans that slow progression and reduce avoidable hospitalizations.
  • Deliver timely inpatient and outpatient consults so referring physicians receive a clear diagnosis, a next step, and a follow-up owner.
  • Lead dialysis rounds and monthly care conferences that improve adequacy, access outcomes, and patient-reported quality of life.
  • Identify transplant candidates early and move them through evaluation with nephrology, surgery, and social work aligned on one timeline.
  • Close the loop on labs, medications, and transitions of care so no high-risk patient leaves a visit without a documented plan.
  • Teach nurses, advanced practice providers, and trainees through bedside decisions they can apply the same day.
  • Use outcomes data—readmissions, access events, blood pressure control—to refine protocols with the practice leadership each quarter.
