About The Position

Trillium Health Resources is seeking an NC ROOTS Regional Head of Chronic Disease Management Program Development Manager to join their team. This role is responsible for designing, building, and operationalizing a comprehensive, region-wide approach to chronic disease prevention and management under the NC ROOTS Hub. The focus is on high-burden conditions such as diabetes, hypertension, cardiovascular disease, and chronic kidney disease, with an emphasis on integrating clinical care, care management, and community-based supports to improve outcomes and reduce avoidable utilization. As a Registered Nurse (RN) leader, this individual will bring clinical credibility and practical care delivery insight to ensure programs are evidence-based, operationally feasible, and scalable across diverse providers and communities. This recruitment is contingent upon program funding approval, with the position effective June 1, 2026, and the organization reserves the right to delay hiring, modify, or withdraw the position based on funding status.

Requirements

  • Associate’s degree in Nursing and three (3) years of experience in chronic disease management, care management, or population health, including experience designing or implementing care management or chronic disease programs; OR Bachelor’s degree in Nursing and one (1) year of experience in chronic disease management, care management, or population health, including experience designing or implementing care management or chronic disease programs; OR Equivalent combination of education/experience.
  • Fully licensed by the North Carolina State Board of Nursing as a Registered Nurse (RN).
  • Must have a valid driver’s license.
  • Must reside in North Carolina and within Region 2 of Trillium’s catchment area, which includes the following counties: Alleghany, Ashe, Davidson, Davie, Forsyth, Guilford, Randolph, Rockingham, Stokes, Surry, Watauga, Wilkes, and Yadkin.

Nice To Haves

  • Master’s degree in Nursing, Public Health, Healthcare Administration, or a related Health or Human Services field.
  • Supervision of two (2) or more employees.
  • Experience in Medicaid populations and/or rural health settings.
  • Experience working across healthcare systems and community-based organizations.
  • Experience with quality improvement methodologies (e.g., Lean, Six Sigma, PDSA).
  • Familiarity with North Carolina healthcare landscape.

Responsibilities

  • Lead development of a regional chronic disease management strategy aligned with NC ROOTS priorities.
  • Design workflows that reflect real-world care delivery, including care management and care coordination models, patient education and self-management support, medication adherence strategies, and transitions of care (e.g., hospital to home).
  • Align chronic disease programs with community-based services, including Community Health Workers (CHWs), Nutrition and Food is Medicine programs, and social services addressing social drivers of health.
  • Define and track key performance indicators, including disease control measures (e.g., A1c, blood pressure), hospitalizations and ED utilization, medication adherence, and patient engagement and retention.
  • Identify gaps in care management and chronic disease workforce capacity.
