Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency operating throughout Arizona, providing residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is a premiere non-profit provider utilizing cutting-edge, nationally recognized, evidence-based models for prevention, education, and treatment. The Navigator II's primary role is to enhance the likelihood of success for clients, identify and reduce barriers, and connect them with needed services and resources. This role involves working with individuals requiring additional support and engagement, conducting outreach, engagement, case management, and providing ongoing wrap-around supportive services. The Navigator II will also map out appropriate next steps for individuals needing assistance in accessing various services to achieve ongoing success. This position is part of a multi-disciplinary team and requires documentation and tracking of patient interactions according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Policies and Procedures, and specific program requirements.
