About The Position
Community Hospice is looking for a qualified Music Practitioner/Therapist to join our team! The Music Therapist will provide complimentary therapy services to all levels of care (hospitals, nursing homes, and patients' homes). The Music Therapist is an important member of our health care team, serving Hospice patients in Home Care, Nursing Home or Inpatient settings.
Requirements
- A current license to practice as a Music Therapist Or Certified Music Practitioner in the state of New York required
- Possession of a valid New York State driver’s license and available automobile for travel throughout the service area.
- Experience working with computers, including laptops, email, cell phone and basic software programs along with confidence in providing electronic clinical notes and documentation regarding patient sessions.
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
Nice To Haves
- Prefer two years professional experience in music discipline; previous home care, hospice or health related experience important.
Responsibilities
- Responsible for direct care and evaluation of patients, with resultant formulation and implementation of treatment program as appropriate to the specific needs of that patient
