About The Position

Community Health and Wellness, LLC offers top-notch services for individuals with disabilities throughout Texas. We have current openings in San Antonio, TX. We are seeking an Independent Contract Music Therapist to provide one-on-one Music Therapy services to children and youth with congenital disorders/intellectual disabilities/physical disabilities. This role involves creating tailored goals, documenting sessions, communicating with team members and service providers, and submitting timely documentation including weekly invoicing, progress notes, and related paperwork.

Requirements

• Must have access to a private personal laptop or desktop computer for HIPAA compliance
• Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader
• Resume
• MT-BC
• Liability Insurance (upon hire)
• Valid Driver's License
• Social Security Card

Responsibilities

• Provide one-on-one Music Therapy services to children and youth with congenital disorders/intellectual disabilities/physical disabilities
• Create tailored goals to meet the needs of individual clients
• Document on each session
• Communicate with other team members and service providers
• Turn in timely documentation including weekly invoicing, progress notes, and related paperwork
• Travel to current or prospective clients’ homes
• Communicate with health care professionals, clients, families, and the general public through verbal and written communication
• Implement patient care plans through home and community goal-related activities
• Work independently, manage one’s own time and the time of client(s)
