About The Position

St. Luke's is seeking a full-time Music Therapist/Case Manager to assist in the emotional and mental rehabilitation or adaptation of individuals with disabilities using music therapy skills and other creative modalities. As a member of the treatment team, under the direction of the physician and/or Clinical Supervisor, this role aids in the assessment, treatment, and discharge planning of individuals with disabilities. The position requires creativity, adaptability, and the ability to make independent judgments. The music therapist serves as a case manager, coordinating treatment for an assigned number of patients.

Requirements

  • Four year bachelor degree from an accredited music therapy program.
  • Current Membership through the American Music Therapy Association.
  • Certification through the Certification Board of Music Therapists.
  • Six months to one year experience in psychiatric music therapy including an awareness of community resources and current therapeutic techniques to fulfill case management duties.
  • Hold current CPR certification.
  • Ability to be creative, adaptable and make independent judgments.

Responsibilities

  • Provides structured music therapy groups, individualized treatment programs and develops new programs.
  • Provides psycho educational information to patients and their families.
  • Timely documentation of patient related information.
  • Participates in and presents in-services to behavioral medicine staff.
  • Co-supervises students to assure compliance with hospital and departmental policies.
  • Represents allied therapy in various multidisciplinary meetings.
  • May provide back-up for department therapists during vacation periods or extended absences.
  • Provides case management duties as assigned by Supervisor and/or attending physician.
  • Case Management duties include individual assessments, utilization reviews, family or support interventions and assistance with discharge planning.
  • Performs admission review on assigned patients.
  • Participates in individual patient care meetings with multidisciplinary team members and the patient/family to evaluate progress and to identify and resolve problems that may interfere with a positive patient outcome.
  • Assesses discharge planning needs to ensure a safe, timely and efficient discharge.
  • Identifies variances during the patient's stay in order to evaluate and improve processes that affect the efficiency and quality of patient care.
  • Arranges for community services (including short and long term placement) prior to discharge to meet patient's needs with recognition and documentation of patient choice of service providers.
  • Demonstrates effective use of these resources within established reimbursement guidelines.
  • Provides patient education and advocacy as appropriate.
  • Maintains knowledge and awareness of mental health laws regarding patient's rights and voluntary/involuntary commitment proceedings, coordinating such proceedings with the necessary personnel.
  • Maintains appropriate, timely and effective communication with patients, families, community agencies, physicians, nursing staff, department managers, ancillary departments and the Program Manager regarding variances, aspects of patient care management, and interpersonal issues with staff members of departments in order to facilitate a positive outcome for patients.
  • Assists in the utilization management of psychiatric patients.
  • Maintains clinical records of all patient contact and clinical reviews, according to hospital policy and other regulatory guidelines (i.e., State, JCAHO).
