The Child & Family Services Division (CFSD) of the Department of Human Services (DHS) is seeking a Multisystem Youth and Family Case Manager to support children and families who are at-risk of out-of-home placement and multi-system involved, or in need of multi-system collaboration. The primary goal is to maintain youth safely in the community at home or with kin. This employee will be part of a multi-disciplinary treatment team responsible for acting as court liaison to include consultation with judges, Guardian Ad Litem (GALs) and court services staff; facilitating cross divisional and multi-agency staffing's on youth who are multi-system (court, behavioral health, child welfare) involved or at risk of becoming multi-system involved; responding to court notices and requests to the Department, and providing short-term case management. Short-term case management services may include visits to the home, detention center, residential placements, or other community settings.
