About The Position

Assesses and evaluates patients/families' social, psychosocial and environmental situations to determine patient needs. Demonstrates knowledge of the disease entity and common medical interventions in the service area assigned.

Requirements

• Masters in Social Work from an accredited school required.
• New York State License (LMSW) required.
• Experienced with acute inpatient, experience discharges planning acute care.
• Knowledgeable SNF and home care referrals.
• Proficient in biopsychosocial assessments.
• Knowledge of pertinent state and federal regulations regarding social work and discharge planning, familiarity with post-acute provider services, financial screening, and reporting requirements.
• Minimum 1-2 years experience in an acute care hospital required.
• Effective interpersonal, communication and organizational skills required.
• Computer proficiency required.
• Ability to problem solves using critical thinking required.

• Geriatric experience.

Responsibilities

• Licensed Masters Social Work to provide services including discharge planning for acute care population.
