Requirements

  • Graduate of an Institution certified by the Council of Social Work Education with a Master's degree in Social Work.
  • (LMSW) LIC MASTER SOCIAL WORKER by the Texas State Board of Social Work Examiners. Temporary license is acceptable during first 90 days of employment.
  • (CPI) CRISIS PREVENTION INSTITUT CER - training must be completed within 30 days of hire date.

Responsibilities

  • Participates in multidisciplinary rounds and proactively determines patient needs for discharge transitions including transportation, DME, LTAC, SNF, home health agencies and hospice.
  • Utilizes crisis intervention and problem-solving skills to facilitate and enhance patient and family's ability to meet acute and ongoing medical and psychosocial needs.
  • Demonstrates ongoing familiarity with community resources to facilitate positive patient and family outcomes agencies to assure appropriate client access.
  • Maintains effective documentation including tracking and referral information; consistently meets department and UTSW documentation standards.
  • Identifies and reports situations or cases to appropriate governmental agencies as legally required and in compliance with organizational policies and procedures.
  • Serves as an active participant of the health care team, communicating effectively with other health care providers, patients, families, other hospital personnel, and visitors.
  • Identifies the need for professional growth and seeks appropriate development opportunities.
  • Provides education to staff, patients and community resource affiliates.
  • Maintains system for tracking placement of patients after discharge on monthly basis.
  • Has knowledge of Department and Unit Performance Improvement program and is aware of expected participation.
  • Performs other duties as assigned.
