As a MOUD Case Manager, you will exercise discretion and judgement in applying your knowledge in aiding individuals and families requiring assistance dealing with substance use/misuse in an outpatient setting. Under regular supervision of a Program Manager, you will provide compassionate and appropriate care and case management for an adult patient population. A MOUD Case Manager will typically carry a caseload of 30+ patients so time management, organizational skills, and prioritization skills are paramount to being successful in this role. Requirements

  • Bachelor’s degree in human service field and experience performing the essential duties of the position are required.
  • A MOUD Case Manager will typically carry a caseload of 30+ patients so time management, organizational skills, and prioritization skills are paramount to being successful in this role.
  • Demonstrates a high degree of personal ownership and accountability, strong work ethic with a passion and commitment to our patients and their treatment journey.

Nice To Haves

  • Master’s degree in human service field and experience in performing the essential duties of the position are preferred.
  • Licensure as a LPC, LMSW, LISW, or LMFT and/or certification as an ADC or AADC are highly desired.
  • Certified as Alcohol and Drug Counselor through Addiction Professionals of South Carolina, or able to complete the APSC certification process within the required timeframe.

Responsibilities

  • Provides case consultation and case management services to assure comprehensive quality care of a patient.
  • Manages and monitors patient progress, identifies patient needs, and maintains all clinical records through independent work as well as treatment team engagement for MAR patients.
  • Provides assessments as needed.
  • Participates in regular clinical supervision.
  • Provides individual, family, group and crisis intervention counseling services.
  • Completes GPRA, GPRA follow ups, and behavioral health screens for MAT patients.
