About The Position

Mosaic at Home Coordinator/Case Manager: Empowering people with intellectual and developmental disabilities to live meaningful lives in caring communities. This role involves building connections with individuals supported, their families, host home providers, and direct care workers. The Coordinator acts as a bridge builder, ensuring individuals have a voice and personalized support. Responsibilities include developing relationships, helping individuals express goals, monitoring relationships between individuals and home providers, creating Individual Support Plans, overseeing service delivery through home visits, and providing on-call support to providers and direct care workers.

Requirements

• A Bachelor’s degree in human services or related field is preferred.
• A minimum of two years of related work experience.
• A valid driver's license in good standing.
• Top-notch communication skills.
• A knack for relationship-building.
• Excellent problem-solving abilities.
• Computer proficiency.
• Experience in behavior management.
• A good grasp of HCBS industry regulations.

Nice To Haves

• Previous experience working with people with intellectual disabilities is a huge plus.

Responsibilities

• Build strong relationships with individuals supported, their families, host home providers, and direct care workers.
• Help individuals express their goals, dreams, and needs.
• Monitor relationships between individuals and their home providers to ensure positive and supportive matches.
• Collaborate with the service planning team to create Individual Support Plans, balancing individual wishes with policy and regulatory requirements.
• Oversee service delivery and contract compliance through regular home and service environment visits.
• Provide on-call support to providers and direct care workers.
