About The Position

Eliot is seeking a Case Manager to join their team in providing Respite services to adults with serious and persistent mental illnesses. Eliot’s Respite program is a short term restorative service that assists clients with obtaining and maintaining community living. As a Case Manager, you will be responsible for providing case management and coordination for 8-12 consumers. Our ideal candidate will have the skills to establish supportive relationships, excellent communication, crisis management and problem solving skills.

Requirements

Requires a Bachelor’s degree or Master's degree in Psychology, Social Work or related field
At least 2 years of experience working with adults with mental illness
Valid Massachusetts drivers license

Responsibilities

Provide intensive and mobile case management, care coordination and support and interventions to assigned consumers either in the community or at site-based Respite.
Coordinate and facilitate services which may include assisting consumers with managing income, insurance, other entitlements, accessing other services and community resources, and scheduling/coordination of all medical and psychiatric appointments.
Develop IAP’s (Individualized Action Plans) for each consumer as assigned. Ensure that treatment goals are culturally competent, strength-based, realistic, attainable, and promote recovery.
Provide support for assigned caseload, including advocacy and assistance with IAP goals.
