About The Position

The MIH Clinician provides front-line in-home and community-based outreach, assessment, and care coordination to individuals with complex needs, particularly those impacted by substance use disorder, behavioral health challenges, and unstable living conditions. The primary objective of the MIH Clinician is to support clients in achieving better health and stability through proactive care and help filling the gaps between community resources. This position is currently grant funded.

Requirements

  • Current resident of the State of Wisconsin at the time of application.
  • Valid Wisconsin driver’s license maintained throughout employment.
  • High School Diploma or GED equivalent
  • At least one (1) year of EMS experience working with populations impacted by Substance Abuse Disorders (SUD), Behavioral Health Crises or related field.
  • Intermediate knowledge of all related computer and software applications, such as word processing and spreadsheets, ImageTrend, Zoll X-Series, and medical equipment.
  • Current Wisconsin EMT-Paramedic License
  • Community Paramedic Certification (or completion within 12 months of hire)
  • Available and willing to work, day and evening shifts.
  • Able to lift/carry and push/pull up to 100 lbs.
  • BLS certifications at time of application

  • CEVO/EVOC Certifications
  • Three (3) or more years of EMS experience working with populations impacted by Substance Abuse Disorders (SUD), Behavioral Health Crisis or related field.
  • The ability to integrate with Fire Departments to engage high risk patients with chronic disease and addiction.
  • Support and connect patients to care where fire departments have limitations in their capabilities.
  • Knowledge of Office 365 and how to generate, track and measure care plans are essential to this role.
  • Good communication skills with a variety of populations and literacy levels.

Responsibilities

  • Conduct engagement visits with individuals experiencing substance use disorder, behavioral health crises, high EMS utilization, and unmet social determinents of health. Build rapport and assess immediate needs in homes, shelters, or street settings.
  • Facilitate connections to primary care, substance use treatment, mental health services, housing supports, and community resources. Collaborate with the team social worker and partner organizations to ensure follow-up and continuity of care.
  • Participate in team briefings, case reviews, and ongoing training. Share insights and clinical feedback with peers and the MIH Captain to support continuous learning and improvement.
  • Thoroughly document entire client encounters consistent with expectations of Milwaukee County OEM-EMS. Maintain documentation of encounters, assessments, referrals, and outcomes while tracking key performance indicators for program evaluation.
  • Ensure the cleanliness, stocking, and operational readiness of medical equipment and response vehicles to support safe and effective field operations.
