About The Position

We are seeking independent Registered Nurses to conduct targeted in-home diagnostic visits and biometric screenings. This is a pure clinical screening role to help close care gaps for insurance members— no comprehensive home safety or ADL/iADL assessments required. You will travel locally between patients' homes (averaging 20 minutes of drive time) to perform customized lab collections and portable diagnostic testing.

Requirements

• Active, unrestricted Registered Nurse (RN) license in the state of practice.
• Strong phlebotomy skills and confidence operating portable diagnostic equipment.
• Excellent time-management skills to maintain a steady daily cadence of field visits.
• Valid driver’s license, a reliable personal vehicle, and a clean driving record.

Responsibilities

• Complete 6 to 7 scheduled in-home visits per 8-hour shift using an optimized local routing system.
• Record blood pressure, weight, and oximetry at every visit, charting in real-time via a mobile health platform.
• Handle and label targeted specimens based on member care gaps, including: HbA1C & GFR blood collection, PSA blood screening, FIT Testing (Colorectal), Urine Microalbumin (Kidney health).
• Operate specialized mobile equipment to conduct: Bone Density Scanning, Automated Diabetic Eye Exams, PAD Testing (Peripheral Artery Disease).
