About The Position

Seacoast Mental Health Center is seeking a passionate individual to join their nonprofit organization as a Mobile Crisis Clinician in Emergency Services. This role is crucial for providing 24-hour crisis intervention and stabilization to individuals and families in the community, aiming to prevent suicide, severe debilitation, and violent behavior, while reducing the need for long-term hospitalization. The clinician will work collaboratively with other Emergency Service members and multidisciplinary agency staff, ensuring seamless patient transitions when higher levels of care are needed, particularly with embedded clinicians at Exeter Hospital.

Requirements

  • Master's Degree in a clinically related mental health discipline.
  • Significant experience in clinical diagnosis and therapeutic intervention.
  • Ability to form rapid rapport and interact with a broad range of clients, including severe and chronic populations.
  • Ability to interface with various community agencies/organizations and multidisciplinary staff.
  • CPR and First Aid Training recommended.
  • Completion of a 40-hour crisis response training that includes concepts and topics in accordance with the National Guidelines for Crisis Care Best Practice Toolkit.
  • Proficiency with the Columbia Suicide Severity Rating Scale.

Responsibilities

  • Provide phone, in-person, or telehealth crisis intervention, assessment, and stabilization services in various settings, including patient homes and community locations, during and after agency hours.
  • Offer short-term crisis counseling and case management as needed.
  • Participate in 24-hour crisis services (phone and face-to-face) at The Exeter Hospital.
  • Assess client crisis states, immediate needs, urgency levels, and determine appropriate intervention modes based on suicide risk, violence potential, coping abilities, mental status, support systems, and need for medication, crisis housing, brief hospitalization, IEA, or outpatient treatment.
  • Manage the acute phase of behavioral crises for youth, adult, and geriatric populations.
  • Provide consultation to staff at local homeless shelters.
  • Independently or collaboratively formulate and initiate plans of action with clients, psychiatrists, case managers, and other staff.
  • Effectively utilize supervision by sharing pertinent information and accepting feedback for clinical improvement.
  • Assist colleagues to optimize patient care delivery while maintaining professional demeanor.
  • Communicate client crisis status, interventions, follow-up needs, and treatment planning to appropriate staff, including developing and using Emergency Services treatment plans.
  • Perform case management and psychotherapy functions during crisis stabilization, focusing on returning patients to their baseline by connecting them with agency or community services.
  • Provide consultation and/or evaluation to Exeter Hospital Emergency Room physicians and staff for psychiatric emergencies.
  • Conduct telephone consultations with Portsmouth Regional Hospital Emergency Room for Seacoast clients in crisis or requiring Conditional Discharge Revocation.
  • Consult and evaluate individuals presenting in other areas of Exeter Hospital.
  • Complete Conditional Discharge Revocation at Exeter Hospital or Portsmouth Regional Hospital when ordered by an agency psychiatrist or APRN.
  • Provide emergency consultations, information, and evaluations to community agencies such as police, jails, foster homes, and homeless shelters, potentially including out-of-catchment interactions.
  • Consult with Seacoast Lodge Staff regarding residents in crisis, offering support and conducting crisis evaluations when necessary.
  • Transport clients to emergency departments, crisis centers, or peer support when clinically appropriate.
  • Work non-traditional shifts to support 24/7 program requirements.
  • Maintain accurate and timely records and documentation in accordance with ES and agency policies and procedures.
