About The Position

The Mobile Crisis Case Manager position provides quality and timely case management services through a strength-based approach to clients experiencing a crisis, in support of the client stabilizing from a psychiatric emergency, adapting to life situations, learning independent living skills, and preventing or reducing hospitalizations.

Requirements

  • Associate’s degree plus two years’ experience in the health and human services field OR four years’ experience in the health and human services field is required.
  • Must complete Case Management Training and provide certificate within 6 months of hire.
  • Computer experience in Microsoft Office programs and basic computer fluency.
  • Ability to work with persons with serious emotional disturbance, severe and persistent mental illness, and/or substance abuse issues, especially those experiencing a psychiatric crisis; possess demonstrated interpersonal skills and ability to react effectively in a variety of human service situations; ability to work independently and as a member of a therapeutic team.
  • Language skills are required, including the ability to read, write, comprehend, and speak the English language.
  • This position also requires the ability to interpret written materials, write reports and business correspondence, and effectively present and respond to questions.

  • Bachelor’s degree in psychology, social work, or counseling is preferred.

Responsibilities

  • Provides direct crisis case management services to children and adults experiencing a psychiatric crisis, in an effort to stabilize and de-escalate individuals and integrate or maintain them in the community while working to maintain them in the least restrictive setting, when possible and applicable.
  • Provides first contact with those experiencing a psychiatric emergency, assesses appropriate level of services for individual’s needs, and coordinates needed services with internal service providers.
  • Responds across service area as part of a two-person team, providing mobile crisis interventions to a variety of individuals or families in need.
  • Reviews all cases when clients are released from an inpatient psychiatric hospital or have stabilized after crisis house placement. Consults with emergency services therapist to develop an Alternative Safety Plan or consider client’s return to a secure placement after completing post hospital discharge assessment while out in the community.
  • Assists those in a psychiatric crisis to identify and secure resources within the agency and community.
  • Maintains accurate and complete records of all client services, as defined by state licensing agencies, funding agencies, and Mental Health Center standards and requirements. Submits progress notes, timesheets, and all work-related paperwork in a timely manner and in acceptable format as determined by agency requirements.
  • Develops knowledge of community resources and how to access these services for the benefit of clients.
  • Establishes and maintains a positive working relationship and professional demeanor with clients, external community partners, Center staff members, and visitors.
  • Attends professional workshops and seminars in order to maintain and improve professional skills and meet licensure/certification requirements.
