About The Position

A Midlevel Provider Clinic position at Scotland Memorial Hospital, Family Medicine Department. This is a full-time role with a Monday-Friday, 1st shift schedule. The provider will interpret clinic operating procedures, interview patients, take medical histories, and perform comprehensive and problem-specific history and physical examinations. Responsibilities include developing and maintaining patient records, providing primary care under physician direction, formulating diagnoses, initiating orders, and interpreting lab studies. The role involves managing therapeutic, preventive, and rehabilitative plans, prescribing and adjusting medications according to law and clinic protocols, and preparing requisitions for tests. The provider will take primary responsibility for managing patients with chronic problems, facilitating discussions about healthcare wishes and advance directives, and initiating referrals to specialists or community resources. Health education, counseling, and maintaining current skills through continuing education are also key aspects. Participation in the Scotland Free Clinic and hospital-sanctioned events like sports physicals is expected. Compliance with JCAHO audit processes, maintaining 90% or above on monthly chart audits, is required.

Requirements

  • Practice independently within the legal boundaries established by the laws of the state where the clinic is located.
  • Function under general supervision of a designated physician.
  • Activities will be carried out in accordance with process protocols and standardized procedures developed collaboratively by the FNP/PA and supervising physician.
  • RN’s/LPN’s/CNA’s will perform both verbal and written orders by the FNP/PA.
  • Orders and prescriptions, as annotated in the patient chart, will be countersigned after each visit by the supervising physician.
  • Performs procedures only when specifically trained, certified, and skilled to do so.
  • Shall practice independently to the degree that s/he has the necessary educational preparation, experience and skill.
  • Shall consult with, or refer to the physician and /or other appropriate members of the clinic team, when additional information is required.
  • Staff members will have knowledge of human growth and development through the life span of the patient and have the ability to meet the needs of and provide the care of these patients.
  • The age categories to be included are: Neonatal/Infant 0 - 11 months; Pediatrics 1 – 12 yrs; Adolescence 13 - 17 yrs; Young Adult/Adult 18 – 64 yrs; Adult/Middle-Aged 65 - 79 yrs; Geriatric 80+ yrs.

Responsibilities

  • Interprets nursing/clinic operating procedures and policies and reviews work performed to determine compliance to recognized standards of care.
  • Interviews patients and takes a complete medical history during initial visit.
  • Performs both comprehensive and problem specific history and physical examinations and provides routine, ongoing care and management of common health problems.
  • Develops and maintains the H & P, problem list, medication list, notes, and other assessment instruments that may be used.
  • Provides appropriate primary care to patients in accordance with approved protocols and under the direction of the supervising physician.
  • Formulates diagnoses and problem list, initiates orders and interprets common lab and diagnostic studies, develops, initiates, and monitors the therapeutic, preventive, and /or rehabilitative plan within established protocols and with supervision and approval of physician.
  • Furnishes/prescribes and adjusts medications according to the law and clinic protocols.
  • Prepares requisitions for special tests and procedures and when required assists patient with preparation.
  • Takes primary responsibility and provides ongoing evaluation and management services for a caseload of patients with multiple chronic problems who are in need of some level of on-going care.
  • Initiates, facilitates, and documents ongoing discussion of healthcare wishes and medical decision issues such as durable power of attorney for Healthcare and / or Advance Directives.
  • Initiates appropriate referrals to physician specialists or community resources (e.g. Home Health, Physical Therapy, Adult Day Care).
  • Provides health education, both verbal and written as well as counseling to patients, family and care providers in coordination with the other members of the health care team.
  • Maintains current skills and knowledge through continuing education activities.
  • Participates in the Scotland Free Clinic and other hospital sanctioned events (I.e. sports physicals, etc.).
  • Compliant with the JCAHO Audit Process by maintaining 90% or above at all times on monthly chart audit survey.
  • Implements and adheres to the organizational wide policies established by SMH.
  • Thoroughly familiar with the Family Practice Center’s Policy and Procedure Manual, including job descriptions, orientation checklists, and state-required documentation.
  • Understands the Employees Guide of the Hospital.
  • Assist Director of Family Practice Center Operations, (DFPCO’s) with the development of annual capital and working budgets, under direction of the SMH Accounting Department.
  • Insure working budget is not exceeded on a monthly basis.
  • Will report and explain any deviation over projected operating budget by the 5th day after receipt of the responsibility report each month.
  • Assist the DFPCO’s with annual staff evaluation.
  • Insure clinical staff accomplish competency checklists annually and are involved in essential continuing education to enhance skills and knowledge.
  • Assist and train new staff as appropriate.
  • Assist in the hiring, discipline, termination of employees as necessary. In accordance with SMH HR policies.
  • Notify DFPCO’s of reportable incidents occurring on site.
  • Respond to patient complaints as appropriate.
