About The Position

Metropolitan Family Services is seeking a full-time Mental Health Juvenile Justice/Future Leaders Youth Engagement (MHJJ/FLYE) Case Manager to join their Southeast Team. Requirements

• Bachelor's Degree in Psychology, Social Work, or related field from an accredited college or university.
• Satisfactorily pass all relevant screenings and criminal background checks.
• Driving for work required.
• Ability to maintain assigned community and home-based services.
• Ability to work effectively with diverse individuals and groups.

Nice To Haves

• 1+ year of case management experience in a social service or healthcare setting.
• Some direct service experience preferred.
• Transportation of clients preferred in a personally owned vehicle.

Responsibilities

• Conducts thorough assessment of referred and accepted clients, evaluating problem and service needs with client input.
• Develops a written action plan with clients to meet their desires and needs.
• Works with clients to supplement service needs with appropriate wraparound services, referrals, and linkages.
• Provides supportive help, advocacy, case management, and mentoring to individuals and families in coping with environmental and interpersonal relationship issues.
• Works collaboratively with community resources, developing a knowledge of resources and helping clients utilize them, including referrals and linkages for successful program graduation/discharge.
• Works within contract, grant, and program requirements, including meeting timelines and reporting requirements.
• Engages clients in settings that may present challenges for the purpose of learning/practicing/developing healthy behaviors and coping skills.
• Reassesses and monitors client status and the adequacy of the action plan within appropriate and required time frames.
• Collaborates and participates with other program staff in the planning and implementation of action plans.
• Handles emergency and crisis situations within the target population subsequent to consultation with a supervisor.
• Authorizes limited amounts of financial assistance to own clients with supervisory approval.
• Performs other duties as assigned.
