Requirements

  • 4 year / Bachelor’s Degree (required)
  • Cardiopulmonary Resuscitation (CPR) (Required)
  • Current Indiana Driver's License (Required)
  • Licensed as a Physical Therapist (PT) by the Indiana Professional Licensing Agency (IPLA) (Required)
  • Speech Language Pathology (SLP) (Required)
  • Licensed as a Occupational Therapist Registered (OTR) by the Indiana Professional Licensing Agency (IPLA) (Required)

Nice To Haves

  • Master’s Degree: Physical Therapy, Occupational Therapy or Speech (preferred)
  • Two Years of experience as a PT, OT or SLP and management experience. (preferred)

Responsibilities

  • Coordinates the provision of therapy services to patients served by the home care department.
  • Assures communication and compliance with federal, state, local regulations and laws, accreditation standards and agency policy and procedures for each discipline.
  • Demonstrates confidentiality related to patient, employee and agency issues.
  • Informs supervisor immediately of any risk, compliance or liability issues
  • Demonstrates accountability for patient outcome measurements within the agency established goals, the team and individual clinicians.
  • Assures through the coordination and supervision that therapist coordinate and provide services to patients that are high quality, cost effective, and demonstrates appropriate utilization to promote quality patient outcomes.
  • Consistently demonstrates competency in the ability to interpret, use, communicate and comply with applicable federal, state and local regulations, accreditation standards, and agency policies and procedures on an ongoing basis.
  • Assures appropriate staffing and patient coverage through the monitoring and coordination of employee schedules, caseloads, productivity and overtime.
  • Assures that the services provided to patients meets the satisfaction goals established by the agency through the HHCAHPS data.
  • Assures that the services provided to patients follows the standards/guidelines set forth by the agency, follows all physician orders and is within the acceptable standards of practice.
  • Collaborates routinely with the QA RN to determine areas of improvement.
  • Establishes a plan for improvement and reports the progress to the Director on a monthly basis.
  • Demonstrates continued growth in leadership though participation in agency/network leadership seminars or workshops as evidenced by the implementation of ideas/strategies for team growth or improvements.
  • Coordinates and facilitates team case conferences to ensure appropriate utilization and regular team meetings to promote accurate communication.
  • Assures staff meets education/competency requirements as determined by regulation and agency expectations.
  • Demonstrates competency in completion of performance appraisals, appropriate and timely counseling/disciplinary action for supervised personnel according to agency policy and procedure.
  • Participates in the development of team and department budget and strategic plan that promotes effective and efficient delivery of services in alignment with agency and network initiatives.
  • Completion of chart audits, clinical record review, attends committees or meetings and other PI activities as assigned.
