About The Position

Coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated.

Requirements

  • A Master's degree in a social work or social welfare program accredited by the Council on Social Work Education or from a social work or social welfare program offered by a regionally accredited institution.
  • Must be licensed in the state of South Carolina for one of the following: Licensed Independent Social Worker (LISW), Licensed Professional Counselor (LPC), or Licensed Marriage Family Therapist (LMFT).
  • Minimal supervision.

Nice To Haves

  • Licensed Social Worker (LISW): Master's Degree in Social Work or Social Welfare program accredited by the Council on Social Work Education.
  • Licensed Professional Counselor (LPC): Master's Degree in Counseling from an accredited program.
  • Licensed Marriage Family Therapist (LMFT): Master's Degree in Marriage and Family Therapy from an accredited program.

Responsibilities

  • Interviewing clients and/or families to assess the client's current diagnosis, coping strategies, cognitive strengths and deficits, situation strengths and weaknesses and mental status.
  • Developing and discussing proposed treatment plans with the client and detailing the recommendations and the reasoning for them.
  • Monitoring and documenting progress of clients and making service/treatment plan adjustments as necessary.
  • Ensuring established agency procedures are followed and that clinical standards that are set for treatment planning, the delivery of individual case management services, group and psycho-educational data, discharge planning and referral services are met.
  • Planning, implementing and/or coordinating all treatment services delivered to clients.
  • Collaboration and coordination with both internal and external resources are crucial to ensure successful client outcomes.
  • Taking suggestions from team supervisors.
  • Conducts community planning, needs assessment and evaluation concerning societal conditions which impact on social functioning.
