About The Position

This is a full time position with duties being split between two departments, Psych Partial (Req ID 46759) and Case Management (Req ID 45618). The essential functions of this position include, but are not limited to, serving as therapist and Case Manager for patients in the Psychiatric Partial Hospitalization Program. This position also serves as a liaison for therapy services needed by hospital inpatients or emergency room patients who are awaiting placement on a Behavioral Health Unit. The therapist is responsible for planning and delivery of therapy and educational services to patients, and, if indicated, to their families and significant others; monitoring and management of patient behavior in order to maintain the therapeutic environment and patient safety; performs admission, discharge and utilization review; and effectively markets the program. Other functions may be assigned.

Requirements

  • Must have been involved in the delivery of mental health services for at least 1 year within the previous 5 years.
  • Must participate in recipient rights instruction within 30 days of hire.
  • Interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understandable manner.
  • BLS - Basic Life Support
  • FINGERPRIN - Fingerprinting
  • PCSW - Psychologist/Counselor/Social Worker
  • Licensed Master of Social Work, Licensed Master of Psychology or Licensed Professional Counselor in the State of Michigan required. If currently possess a limited MSW license or Professional Counseling license, must obtain full licensure with the state of Michigan within 3 years of hire or transfer required.

Responsibilities

  • Responsible for the assessment, development, implementation, evaluation and documentation of patient treatment programs.
  • Assesses patients upon admission and collects data to determine the current status of patients.
  • Participates improving the patients' physical health and maintaining their safety in all settings.
  • Screens program applicants as needed in all settings and either provides, or makes recommendations, for appropriate treatment and setting. May need to fill in for utilization review as needed.
  • Maintains program patient medical record and records information legibly, accurately, using the approved format, timely, and following all procedures.
  • Participates in the admission process, including orientation of new patients.
  • Provides group or individual therapy for patients when indicated in the partial program or in the other areas of the hospital as needed.
  • Responsible for the documentation of Bio Psycho Social Assessments, Continuing Care Plans, Admission Reviews, Problem Lists, Discharge Summaries and Treatment Planning.
  • May need to fill in for utilization review as needed. Serves as group therapy facilitator for patients, family members and significant others.
  • Conducts group and individual educational sessions for patients, family members and significant others.
  • Provides individual/conjoint therapy to patients, family members and significant others.
  • Participates in patient progress review meetings.
  • Responds appropriately to patient conditions utilizing critical thinking skills.
  • Demonstrates effective and pleasant interpersonal relations.
  • Demonstrates ability to be flexible in changing work environment.
  • Verifies Patients’ insurance, obtains authorization to bill for treatment, and ensures program meets insurance company criteria for reimbursement.
  • Participates in data collection as needed to analyze the success of the Liaison program in addition to the other quality metrics of the program.
  • Adheres to all policies and procedures related to employment, safety, program provision and operations.
  • Is familiar with the Michigan Mental Health Code as it relates to: recipient rights; Confidentiality; Abuse and Neglect; and Duty to Warn.
  • Demonstrates competency in Infection Prevention and Control Practices and Policies.
  • Other duties as assigned.
