About The Position

St. Francis Medical Center is a leading healthcare institution in Los Angeles, providing vital healthcare services to the community. The hospital is recognized for its comprehensive diagnostic and treatment services across various specialties, including Pediatrics and Behavioral Health. St. Francis is a Comprehensive Stroke Center, STEMI Receiving Center, ED Approved for Pediatrics, Geriatric ED, Level III Neonatal ICU, and Level II Trauma Center. This role involves providing emotional support, crisis intervention, and counseling for a diverse patient population, including Pediatrics, Adolescents, Adults, and Older Adults, and their significant others. The position requires case management, psychosocial assessments, participation in treatment plan development, and coordination of discharge planning within a multidisciplinary team. Accurate documentation and maintenance of clinical records are essential.

Requirements

  • Master's Degree in Social Work related field OR Master's Degree in Marriage and Family Therapy or related field.
  • In states without required licensure, then current registration with State Board is required within 3 months of hire.
  • In states with required licensure, license required at time of hire and must be maintained.
  • Behavioral Violence Prevention Training within 3 months of hire and maintain current.
  • Minimum one (1) year hospital or Outpatient experience with emphasis in mental health care.
  • Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.).

Nice To Haves

  • Current BCLS (AHA) certificate upon hire and maintain current preferred.

Responsibilities

  • Provide emotional support, crisis intervention, and counseling for Pediatrics, Adolescents, Adults, Older Adults and their significant others to cope with social and emotional problems.
  • Provide information and referral services.
  • Referrals for financial assistance and supportive services to patients and families with problems related to hospitalization, illness, disability, or trauma.
  • Provide case management and milieu management.
  • Complete psychosocial assessments.
  • Assist in the development and re-evaluation of Treatment Plans.
  • Work as a member of a multidisciplinary team.
  • Coordinate discharge planning.
  • Complete necessary documentation and maintain clinical records for each client.
