About The Position

At Telecare’s AgeWise program, we provide community-based, strengths-based case management and institutional care coordination for older adults (aged 59+). As a Mental Health Rehabilitation Specialist, you aren't just managing a caseload; you are a vital advocate helping individuals recover their health, hopes, and dreams while ensuring they live in the least restrictive environment possible. In this role, you will utilize Psychiatric Rehabilitation principles, Recovery Philosophy, and Trauma-Informed Care to provide direct community support and integrated whole-person care. Comprehensive Case Management: Conduct assessments, provide crisis intervention, and collaborate with members and their families to build highly collaborative treatment plans. System Advocacy & Collaboration: Partner closely with vital community agencies, including the Conservator’s Office, Public Defender’s Office, Protective Services, Probation, and county mental health services. Continuum of Care: Support members living independently by linking them to housing, medical care, and psychiatric services, while also providing discharge planning and advocacy for conserved members in long-term care facilities.

Requirements

  • Master’s degree in Social Work, Psychology, Counseling, Human Services or Social Services and two (2) years of experience providing direct services in Mental Health; or Bachelor’s degree in Social Work, Psychology, Counseling, Human Services or Social Services and four (4) years of experience providing direct services in Mental Health; or an Associate’s Degree and six (6) years of experience providing direct services in Mental Health
  • Valid and current driver’s license
  • Personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
  • Willingness to use your personal vehicle to attend meetings, etc.
