About The Position

As a Mental Health Professional, you will be responsible for maintaining accurate and timely case records in compliance with licensing and Medicaid standards. Your role involves providing direct support during crisis situations, coordinating and developing Individualized Service Plans with team members and stakeholders, and delivering person-centered, trauma-informed, and psycho-educational interventions. You will facilitate individual, family, and group counseling sessions, utilizing your approved credentialing and skill set. Collaborative communication with case professionals such as school personnel, social services, and juvenile justice representatives is essential for cohesive client care. Additionally, you will provide case updates and presentations in formal settings like FAPT meetings, IEP sessions, and court appearances.

Requirements

  • Bachelor’s degree or higher in a related field.
  • At least 1 year of experience working with the identified population.
  • Current QMHP-T (A/C), QMHP-A, QMHP-C, or QMHP-A/C certification, or provisional/full licensure (e.g., Resident in Counseling, Supervisee, Associate license).

Responsibilities

  • Maintain accurate and timely case records per licensing and Medicaid standards, ensuring continuity and quality of care.
  • Provide direct support during crisis situations, helping clients to navigate and stabilize through effective case management.
  • Coordinate and develop Individualized Service Plans with input from team members and stakeholders to align treatment goals.
  • Deliver person-centered, trauma-informed, and psycho-educational interventions designed to support treatment objectives.
  • Facilitate individual, family, and group counseling sessions, utilizing approved credentialing and skill set.
  • Maintain regular contact with case professionals, such as school personnel, social services, and juvenile justice representatives, ensuring cohesive client care.
  • Provide case updates and presentations in formal settings such as FAPT meetings, IEP sessions, and court appearances.
  • Provide individual, family, and/or group therapy sessions
  • Maintain accurate and timely clinical documentation
  • Collaborate with care teams as needed
  • Maintain compliance with all clinical and regulatory standards
  • Participate in supervision (if applicable based on licensure level)
