Mental Health Professional (PLPC, PLMFT, LMSW)
About The Position
Clarvida is seeking a Mental Health Professional (MHP) to join our Assertive Community Treatment (ACT) Team serving adults ages 18 and older with severe mental illness. In this role, you will provide regularly scheduled counseling services and support a caseload aligned with your clinical specialty, helping clients work toward individualized treatment plan goals. ACT services are delivered directly in the community—client homes, places of employment, and other local settings—to support stability, recovery, and long-term independence. You will conduct assessments, provide counseling, coordinate care, and connect clients with community resources aligned with individualized treatment plan goals. You will work closely with a multidisciplinary team to deliver person-centered, evidence-based care while helping clients remain integrated with their families, friends, and communities. This position participates in a rotating on-call schedule (one week every 6–8 weeks). One (1) day of PTO is earned for each on-call rotation, regardless of call volume, to support work–life balance. Assertive Community Treatment (ACT) is one of the most established and widely researched evidence-based practices for adults with severe mental illness. ACT provides comprehensive, person-centered services—including housing support, recovery services, vocational assistance, nursing, psychiatric care, and therapy—directly in the community. The goal is to help clients transition to lower levels of care or increased independence while improving overall quality of life.
Requirements
- Active Louisiana licensure in one of the following: – PLPC -PLMFT -LMSW
- Minimum of two (2) years of professional experience providing services to individuals with serious mental illness
- Valid State Driver’s License in good standing
Responsibilities
- Provide individual and family counseling services to adults with severe mental illness
- Support a clinical caseload aligned with licensure and specialty
- Engage clients through active listening, strengths identification, and therapeutic interventions
- Teach coping skills and support recovery-oriented treatment goals
- Deliver services primarily in community-based settings, with some office-based work
- Collaborate with multidisciplinary ACT team members, including nurses, psychiatrists, and case managers
- Participate in a rotating on-call schedule, earning additional PTO
- Provide client transportation using a company vehicle as needed
- Maintain accurate, timely, and compliant clinical documentation
- Provide case management services aligned with treatment plans
