About The Position

Albany Care, a Specialized Mental Health Rehabilitation Facility (SMHRF), in Evanston, IL, has Immediate Openings for Mental Health Professionals (MHPs) to join our team. These are Full Time Positions with 8 Hour Shifts. Starting Rate of up to $28 per hour, depending on Benefits Selection! Available Position : FT 3pm-11:30 pm Tuesday through Saturday. Mental Health Professionals (MHPs) work in conjunction with an interdisciplinary team, offering recovery and rehabilitation support and services to those who live in the facility.

Requirements

• Bachelor’s Degree in Human Services Field (counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, etc.)
• Bachelor’s degree in other field + 2 years of supervised clinical experience in a mental health setting
• Licensed Practical Nurse (LPN)
• Certificate of Psychiatric Rehabilitation from an Illinois Department of Human Services (DHS) approved program + High School Diploma/GED + 2 years’ experience providing mental health services
• Certified Recovery Support Specialist (CRSS)
• Licensure through the Illinois Alcohol and Other Drug Abuse Professional Certification Association (IAODAPCA)
• Occupational therapy assistant + 1 year experience in a mental health setting
• High School Diploma + 5 years of supervised clinical experience in mental health or human services

Responsibilities

• Collaborate with consumer and treatment team to complete assessments and develop therapeutic treatment plans
• Implement therapeutic care plans and activities in a compassionate and caring manner
• Recognize, note, and inform treatment team when a consumer is not at their baseline of symptoms and behaviors
• Lead groups and hold one-on-one sessions including engagement with evidenced based approaches
• Collaborate with community agencies to coordinate care, including the Moving On Program
• Crisis management and intervention
• Effectively manage a caseload of up to 32 individuals
• Participation in creation of discharge plans and recommendations for other levels of care
