About The Position

The Mental Health Professional (MHP) provides community-based mental health and supportive services to individuals experiencing homelessness, housing instability, and behavioral health challenges. The MHP works as part of the Homeless & Community Support Team (HCST) to assist individuals in achieving housing stability, improving mental health functioning, and increasing independence. Services are primarily delivered in community settings including clients’ homes, shelters, and community locations. The MHP provides clinical support, case management, and skill-building interventions while collaborating with the HCST team to ensure coordinated care. The Homeless & Community Support Team (HCST) provides housing and supportive services to individuals experiencing homelessness and behavioral health challenges. HCST programs focus on housing stability, recovery, and community integration through person-centered and trauma-informed services.

Requirements

  • Bachelor’s degree in Psychology, Social Work, Counseling, Human Services, or a related field required
  • Strong organizational and communication skills
  • Ability to work independently in community-based settings
  • Basic computer skills and ability to complete required documentation

Nice To Haves

  • Experience working with individuals with mental health, substance use, or housing instability preferred
  • Knowledge of recovery-oriented and trauma-informed care approaches preferred
  • Valid driver’s license and reliable transportation preferred

Responsibilities

  • Provide community-based mental health services and supportive case management
  • Assist individuals in developing and working toward individualized service and recovery goals
  • Conduct skill-building interventions including coping skills, daily living skills, communication, and symptom management
  • Assist with client intakes, including gathering documentation, completing assessments, and coordinating enrollment into HCST programs
  • Support individuals in accessing housing resources, healthcare, benefits, and community services, which may include providing transportation
  • Collaborate with team members to coordinate services and support housing stability
  • Attend required team meetings, supervision, and program trainings
  • Maintain accurate and timely documentation including progress notes and client records in accordance with agency and program guidelines
  • Participate in outreach and engagement efforts with individuals experiencing homelessness
