About The Position

The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for an Overnight Mental Health Professional who demonstrates this mission and wants to work for an organization that makes a difference in the community. The MHP is responsible for the implementation of the client’s IM+CAN treatment plan goals and objectives. The MHP demonstrates this through daily documentation of service provided to each client to whom they are responsible. The MHP will assure client rights and dignity while maintaining a clean and safe environment. Be familiar with and maintain all applicable program and agency regulations.

Requirements

  • Bachelor’s Degree in Human Services or related field preferred
  • High School Diploma and 5 years of experience in Social Services
  • Experience working with adults with mental illness preferred.
  • Navigation of stairs
  • No lifting restrictions.
  • Ability to provide services in clients’ homes.
  • Computer including Microsoft Windows applications
  • Copy Machine
  • Telephone with voice mail system
  • Basic household appliances
  • The use of personal automobile, a valid driver’s license, and the minimum amount of liability insurance as defined by AID’s Personnel Policy.
  • Must drive self and clients in personal or agency vehicles.
  • Must acquire and maintain certifications in First Aid, CPR, Non-violent crisis intervention training, CEU’s and other relevant training
  • Evening, overnight, weekend and holiday hours may be required
  • Must be able to drive a passenger vehicle
  • Required to work at multiple locations each shift
  • May require minimal day time hours throughout the month to attend team meetings, clinical supervision and required trainings

Responsibilities

  • Work scheduled hours and be flexible to meet client and program needs. Includes cross training and overnight services in other programs or buildings as assigned.
  • Respond to all incoming calls and assess if services can be provided via telehealth or in person. If a client requires in person assistance, staff will provide in person services at the clients location.
  • Staff will be physically present at each site to do a walk through of the building, attend to any client needs or emergencies each shift, rotating every few hours.
  • Collaborate and communicate with Case Managers and Managers to coordinate client services across programs.
  • Communicate with and coordinate emergency services when necessary such as local fire and police departments
  • Answer all calls to emergency phone and overnight phones as assigned while monitoring and granting building access to individuals when necessary
  • Must meet minimum service hour provision/productivity standards (MRO) monthly
  • Complete and sign all MRO documentation (case notes) every 48 hours in Cx360.
  • Provide group services to clients at each building as assigned by Manager or Director.
  • Complete daily logs to document time spent upload at the end of each shift.
  • Acquire and maintain required trainings and certifications as assigned by Manager.
  • Collaborate and conduct client centered consultation with program manager and other staff as needed.
  • Provide effective services to all assigned clients on a regular basis to ensure services meet clients’ individual needs in line with each IM+CANS.
  • Maintain professionalism and good boundaries when working with clients, coworkers and outside agencies.
  • Attend monthly clinical supervisions per DHS requirement, which may require day time hours.
  • Attend monthly team meetings and be a positive contributor, which may require day time hours.
  • Follow the direction of Managers and Case Managers regarding appropriate client interactions and interventions per clients’ IM+CANS.
  • Assure compliance with all rules, regulations and recommendations per agency, state and federal guidelines.
  • Review and follow updated policies and procedures.
  • Recognize emergency situations and take appropriate action. Contact manager and director per procedure.
  • Complete necessary paperwork correctly (incident reports, petitions, encounters)
  • Complete monthly house meetings as assigned and correctly complete necessary drills and paperwork.
  • Promotes client wellness through coaching, counseling, consultation, motivational enhancement, and information and referral services to clients facing various health challenges, as needed.
  • Communicates with service delivery partners, which may include Physicians, Nurse Practitioners, Counselors, Pharmacists, Employers, Community Supports, and other Health Professionals, as needed, to facilitate the achievement of client’s wellness objectives.
  • Utilizes a variety of appropriate resources and educational materials for wellness coaching. (ie., Health Matters Curriculum, Smoking cessation materials, Weight Management information, etc.)
  • Assure client records are properly maintained per agency procedures including scanning necessary paperwork into Cx360 per procedure.
