About The Position

This position serves as the Center's Housing Coordinator to ensure patient access to appropriate community-based housing. The role involves completing assessments of patients living in Community Care Residential Facilities (CRCF's) and assessing the patient's psychiatric status to determine the ability to move to a lower level of care in the Housing continuum. The position is located at the Department of Mental Health, Charleston Dorchester Mental Health Center, 2100 Charlie Hall Blvd, Charleston, SC 29414.

Requirements

  • A master's degree in Guidance, Counseling, or the Social Sciences
  • Professional experience in human services or social service programs.
  • Licensed or eligible for licensure in SC and willing to work towards licensure within the first six (6) months of employment.
  • Must be able to meet BHDD-OMH credentialing requirements.
  • Ability to lift 5lbs.
  • Drive a state vehicle.
  • Able to work a flexible work schedule.
  • Services are delivered in the community and the patient's homes.
  • Knowledge of psychopathology and client management with the community-based psychiatric setting.
  • Experience working with chronically mentally ill adults and seriously emotionally disturbed children and youth.
  • Maintain organizational and communication skills required for this position.
  • Considerable knowledge of methods and practice management principles.
  • Ability to interpret and apply rules and regulations.
  • Knowledge and ability to establish and maintain effective working relationships.

  • Bilingual in English and Spanish (or another language) is a plus.

Responsibilities

  • Provide oversight for the Center's housing programs, including Community Housing, Home share, Home share Respite, TLC/Housing and Homeless Program Supportive Housing developments, Community Residential Care Facilities (CRCFs), and other supportive housing programs, as applicable.
  • Coordinate administrative requirements of the agency's housing programs, including payment contracts and patient agreements, outreach and recruitment efforts for Home share providers, and completion of assessment for patients living in CRCFs.
  • Assess patients psychiatric status to determine ability to move to a lower level of care in the Housing continuum.
  • Coordinate with the agency's Transition program staff, including Clinical Care Coordinators (CCCs) and Transition specialists, MCO CCCs, Certified Peer Support Specialists, and Benefits/Entitlement Specialists.
  • Partner with the local HUD Continuum of Care, public housing authorities, and affordable housing owners and property managers.
  • Coordinate patient housing referrals and move-ins and maintain wait lists for each Center housing program.
  • Enter and maintain Community Housing data in the agency's Electronic Health Record.
  • Prepare and submit monthly Community Housing report.
  • Enter and maintain Center Home share program and patient information in the agency's SharePoint site and CIS/Electronic Health Record.
  • Participate in other OMH or partner agency programs and activities as may be required.
