This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Berkeley Community Mental Health Center, 403 Stony Landing Road, Moncks Corner, SC 29461. As a Mental Health Professional I - ACT, you will serve on the Assertive Community Treatment (ACT) team providing evidence-based services (safe, effective, and measurable) to promote and support the recovery of persons with mental illness.

Requirements

  • A master's degree in guidance, counseling, or the social sciences or related field that qualifies for licensure as an LPC or LISW.
  • Must meet ACT fidelity requirement for Mental Health Professional.
  • Must be able to lift 5lbs and operate a state vehicle.
  • Position is considered essential and may be called back during crises/disasters/emergencies.
  • Position will be a part of the Assertive Community Treatment Team.
  • Requires flexible schedule.
  • Services are delivered in the community and patients' homes.
  • Have practical therapeutic knowledge of principles, methods and counseling practices and procedures for the diagnosis and treatment of substance use disorders and mental health disorders.
  • Have knowledge/skills to conduct comprehensive mental health assessments, formulate diagnoses, initiate treatment plans, and deliver an array of therapeutic services.
  • Have the ability to function in CMH setting.
  • Have the ability to work with a multi-disciplinary team and liaise with community.
  • Have the ability to communicate effectively, orally and in writing.

Nice To Haves

  • Bilingual abilities in English and Spanish (or another language) are a plus.

Responsibilities

  • Deliver services in the community and patients' homes.
  • Attend to daily schedule; Ensure patients are seen as per LOC guidelines; Address patient barriers to services; Engagement completed per procedure; Ensure crises are staffed; Satisfaction Survey scores are positively ranked; Cooperate with community partners, ensures internal & external customer satisfaction.
  • Serve as a required MHP for the ACT team, providing mental health counseling to a population of seriously and persistently mentally ill patients in the community.
  • Meet patient care hours standards.
  • Provide mental health services to ACT beneficiaries.
  • Develop Personal Safety Plans; Protect confidentiality.
  • Record notes will be written collaboratively; Documentation completed according to QI policies; Complete outcome measures per procedures; Services and contacts to patients are documented; Keep billing disallowances to a minimum.
  • Adhere to Center, OMH, & State Corporate Compliance policies, procedures, and responsibilities; Make good faith effort to report violations; Will not intentionally violate laws regulating the provision of health care; Adhere to Code of Conduct.
  • Participate in ongoing professional development; Maintain license/certificates; Comply with training requirements; Attend scheduled supervision, treatment team, and other assigned meetings as required; Utilize leave time according to procedure; Follow chain of command.
  • Help ensure the patient care area and Center maintains CARF accreditation.
