Vancouver Coastal Health (VCH) is seeking a Mental Health Outreach Nurse (RN/RPN) to join the Cambie Older Adult Mental Health and Substance Use team in Vancouver, BC. This position involves working with older adults experiencing dementia, major psychiatric disorders, concurrent disorders, and/or age-related issues. The role requires supporting individuals at risk due to psychiatric symptoms, complex medical/social factors, or significant caregiver stress. Duties include conducting psychogeriatric assessments, providing crisis intervention, case coordination, treatment planning, consultation/liaison, facilitating individual/group sessions, individual counseling, and offering education and support to ensure integration and continuity of care across the care continuum. The nurse will consult with general practitioners, caregivers, and family members, and collaborate with staff in continuing and residential care facilities, as well as hospitals. Responsibilities also include coordinating community resources and delivering educational programs. The role involves establishing and maintaining relationships with clients, families, and colleagues to provide appropriate treatment services based on best practices and client needs. Additionally, the nurse will share knowledge and expertise with colleagues through consultations, in-services, and presentations, acting as a clinical resource for other healthcare providers. The position may involve direct care and outreach services, practicing within a client and family-centered care model, and adhering to the standards of practice and code of ethics set by the British Columbia College of Nurses and Midwives (BCCNM). Practice will align with VCH policies, guidelines, and clinical practice documents, as well as the organization's vision, mission, goals, and values.
