About The Position

One Love Agency is seeking a compassionate, patient-centered Mental Health Medication Management Provider to join our multidisciplinary behavioral health team. The provider is responsible for assessing, diagnosing, and treating individuals with mental health and co-occurring substance use disorders through evidence-based medication management. Working collaboratively with therapists, case managers, CBRS providers, peer support specialists, recovery coaches, and administrative staff, the provider develops and implements individualized treatment plans that promote recovery, stability, and improved quality of life. The provider will also collaborate with the Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) teams to ensure coordinated, timely, and effective medication management for patients receiving higher levels of behavioral health care. The ideal candidate is committed to delivering compassionate, evidence-based care while fostering meaningful therapeutic relationships and supporting long-term recovery.

Requirements

  • Current unrestricted Idaho license (or ability to obtain licensure prior to employment) as one of the following: Psychiatric Mental Health Nurse Practitioner (PMHNP), Physician Assistant (PA-C) with psychiatric experience and appropriate supervising physician relationship, if applicable, or Psychiatrist (MD or DO).
  • Current DEA registration with prescribing authority.
  • Ability to prescribe controlled substances in accordance with federal and Idaho regulations.
  • Strong knowledge of psychopharmacology and evidence-based treatment for mental health and substance use disorders.
  • Excellent clinical judgment, communication, interpersonal, and organizational skills.
  • Ability to work independently while collaborating effectively within a multidisciplinary treatment team.
  • Proficiency in electronic health record documentation.

  • Experience treating co-occurring mental health and substance use disorders.
  • Experience providing Medication-Assisted Treatment (MAT).
  • Experience with Spravato (esketamine) treatment.
  • Experience working in PHP and/or IOP settings.
  • Experience providing telehealth services.
  • Experience working with Idaho Medicaid, Medicare, and commercial insurance plans.
  • Bilingual skills are a plus.

Responsibilities

  • Assess patients' mental health symptoms, clinical history, and current functioning to establish appropriate mental health and substance use disorder diagnoses.
  • Develop and implement individualized treatment plans based on each patient's clinical needs and recovery goals.
  • Provide ongoing medication management services for individuals with mental health and co-occurring substance use disorders.
  • Prescribe, monitor, and adjust psychiatric medications within the provider's scope of practice.
  • Evaluate medication effectiveness, monitor for adverse effects, and manage potential medication interactions.
  • Provide Medication-Assisted Treatment (MAT), including buprenorphine (Suboxone) and other evidence-based medications for substance use disorders, when appropriately credentialed.
  • Provide medication management services for patients receiving Spravato (esketamine) treatment in accordance with REMS program requirements, manufacturer guidelines, and agency protocols.
  • Order, review, and interpret laboratory tests and diagnostic studies as clinically indicated.
  • Educate patients and, when appropriate, their families regarding diagnoses, medications, treatment options, expected outcomes, and potential side effects.
  • Collaborate closely with therapists, case managers, CBRS providers, peer support specialists, recovery coaches, primary care providers, pharmacies, hospitals, and other community partners to coordinate comprehensive patient care.
  • Provide medication management services for patients enrolled in the Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP), ensuring timely medication adjustments and ongoing monitoring throughout treatment.
  • Participate in multidisciplinary treatment team meetings, case consultations, and discharge planning for PHP and IOP patients.
  • Coordinate transitions of care between outpatient medication management, PHP, IOP, hospitals, primary care providers, and community resources to promote continuity of treatment.
  • Respond appropriately to patient safety concerns by completing risk assessments, developing safety plans, and facilitating referrals to higher levels of care when clinically indicated.
  • Maintain accurate, complete, and timely documentation within the electronic health record in accordance with agency standards, payer requirements, and regulatory guidelines.
  • Utilize the agency's electronic health record (EHR) for documentation, prescribing, communication, and care coordination.
  • Provide services through both in-person and telehealth appointments, as appropriate.
  • Maintain compliance with HIPAA, Idaho licensing regulations, DEA requirements, payer guidelines, REMS requirements, and all agency policies and procedures.
  • Participate in quality improvement initiatives, peer consultation, continuing education, and other professional development activities.
