About The Position

The Mental Health Crisis Case Manager will be responsible for conducting crisis clinical assessments for patients seeking treatment in the Emergency Department at Swedish Hospital. This role involves assessing clinical needs, obtaining diagnostic impressions, and collaborating with physicians. The case manager will also assess patients' vital signs, weight, blood alcohol levels, and BMI. A key part of the role includes reviewing previous documentation for case formulation, providing appropriate treatment referrals based on the patient's medical and psychiatric status, and identifying potential issues that could negatively impact the patient or department. The position also requires obtaining collateral information from families and support systems, consulting with multidisciplinary treatment teams, and completing admission paperwork and patient rights documentation. Coordination with families and community agencies, participation in care conferences and huddles, and facilitating bed management planning are also essential functions. Additionally, the case manager will educate patients and families about illness effects and follow up on documentation with local and state agencies as a mandated reporter.

Requirements

  • Master’s degree or higher in Behavioral Health field
  • Current State of Illinois Registered Nurse or Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Clinical Professional Counselor (LCPC), Licensed Marriage and Family Therapist (LMFT), or Current State of Illinois Registered Nurse.
  • Experience with group, individual, and family therapy.
  • Experience with adolescents and families in crisis.
  • Familiarity with Cognitive Behavioral Therapy and Dialectical Behavioral Therapy.
  • Minimum of two years of experience in crisis/emergency services, behavioral health setting preferred.
  • Knowledge of patient rights and law pertaining to mental health
  • Emergency Room mental health therapy

Responsibilities

  • Conducting crisis clinical assessments for patients seeking treatment.
  • Assessing patients' clinical needs and obtaining initial diagnostic impressions.
  • Assessing patients including vital signs, weight, blood alcohol and calibration of BMI for specific population.
  • Reviewing previous documentation prior to assessment process and “case formulation”.
  • Providing treatment referrals consistent with the customer’s medical and psychiatric status and symptoms.
  • Identifying potential/actual problems that may adversely affect the patient/department and reporting them immediately.
  • Overseeing follow up with referral sources as appropriate.
  • Obtaining collateral with families and support systems through face-to-face interviews and/or phone calls.
  • Consulting with multidisciplinary treatment team members on the assessment, treatment, and discharge planning of patients with mental health problems.
  • Completing and acting as a resource to hospital staff on admission paperwork and patient rights.
  • Contacting and coordinating with families and community agencies as appropriate.
  • Participating in care conferences and emergency department huddles to discuss key social, financial, and discharge planning factors.
  • Facilitating bed management planning and coordination with Endeavour Inpatient Psych units.
  • Educating patients and patient families regarding the effects of illness and disability on personal adjustment, family dynamics, relationships, social issues and the like.
  • Following up on documentation with local and state agencies as a mandated reporter per HIPAA guidelines.
