About The Position

The Doe Fund is seeking a Case Manager/Mental Health Counselor for The Doe Fund's Webster permanent supportive housing program for homeless men and women living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. Half of the residents will be referred by the New York City HIV/AIDS Services Administration (HASA), and the other half will be apartments for affordable to low-income households. The Case Manager/Mental Health Counselor is responsible for providing supportive case management services to the HASA-funded residents. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV/AIDS population.

Requirements

  • Master’s Degree in Social Work or related field
  • License in Clinical Social Work and LMSW or LMHC required
  • At least one year of experience working with people who are homeless, living with HIV/AIDS, have substance abuse histories, or suffer from mental illness
  • At least 5 years of experience in the mental health field
  • Knowledge of benefits in the New York City Metro area, HIV/AIDS, psychiatric diagnoses, substance abuse, and mental health issues
  • Solid computer skills – internet, Microsoft Office, and charting database
  • Great organizational, writing, and communication skills

Responsibilities

  • Maintain a caseload and provide regular case management services to these individuals
  • Maintain resident records in compliance with agency and funding standards
  • Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self sufficiency
  • Monitor and ensure residents are linked to healthcare providers
  • Provide counseling and crisis intervention as necessary
  • Assist residents in securing and maintaining benefits
  • Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA and mental health programs
  • Facilitate groups and activities that promote health and independent living skills development
  • Monitor and assist residents with rent payments and budgeting
  • Conduct regular unit inspections
  • Educate residents about approaches to mental health
  • Attend ongoing trainings to enhance the services provided to the residents
  • Other duties as assigned
